Institution of Decree No. 80,281, of September 5, 1977, medical residency is a modality of post-graduation teaching for physicians, based on in-service education. Ethical issues, medical-legal, as well as changes in current medical practice, lead the demand of improvement on the training and assessment in residence. We performed a project to adaptate and introduce an instrument for objective evaluation of surgical practice, at the otolaryngology department of Hospital Onofre Lopes (HUOL). The chosen instrument was the 'procedure based assessment' used in residence programs of United Kingdom . The purpose of the evaluation is to allow a standardization and improve the teaching-learning process of surgeries performed in this area, allowing an evaluation of the resident progress and also the identification and correction of weaknesses in this process. This is an exploratory, prospective and descriptive study carried out with all residents of otolaryngology at HUOL. The project was performed in three stages, the first consisting in the translation and adaptation of the instrument. The second, in the application of the evaluation instrument and analysis of the data collected. The third, in the application of a questionnaire for an evaluation of the residents experience with the use of this instrument. 18 PBAs were applied in the evaluation period. It was possible to verify in what degree of competence the students were during the training. The instrument shown to be useful to emphasize the points developed and the difficulties to be overcome in the surgical training. The instrument was well accepted and demonstrated to be a useful tool in improving the surgical training process in our service of otolaryngology residence in SUS.
