SUMMARY:

This thesis analyzes the impacts of the National Program for Improvement of Access and Quality of Primary Care (PMAQ-AB) in the municipalities of the state of Pernambuco, based on the perceptions of workers of the Family Health Strategy (FHS) and municipal managers of Primary Care. The study aimed to understand the effects of PMAQ-AB on motivation and professional recognition, organization of work processes, structural conditions of health units and quality of care, and to identify the challenges and limits of policy in the context of municipal management. It is a research of predominantly qualitative approach, with complementary quantitative elements, structured as a multiple case study and presented in the form of two scientific articles.

The study territory included 12 municipalities of Pernambuco, intentionally selected, considering participation in the 2nd and 3rd cycles of the PMAQ-AB, the existence of more than one ESF team, different certification results in the 3rd cycle, diversity of population size and belonging to different health macro-regions. The first article had as subjects 206 workers of ESF, including different professional categories, and used focus groups as main technique of data collection. The discussions were recorded, transcribed in full and supplemented by field diary records. The data analysis was carried out through thematic analysis, allowing to identify patterns related to professional motivation, work organization, unit structure and quality of care.

The results of the first article indicate that workers perceive advances promoted by PMAQ-AB in the organization of work processes, in the planning of actions and in the qualification of assistance, associated with the financial incentive for performance. However, persistent limitations were also identified, especially related to infrastructure, inequality between municipalities and the sustainability of the program’s effects on daily services. The second article, in preparation, analyzes the perception of municipal managers of Primary Care, based on individual interviews and the use of the Collective Subject Discourse, focusing on decision-making processes, the implementation of the PMAQ-AB and the challenges in applying the financial performance incentive.

By integrating the perspectives of workers and managers, this thesis contributes to the deepening debate on policies for evaluation and financing of Primary Health Care in Brazil. The findings highlight convergences and divergences between the different actors involved, offering subsidies for the improvement of management strategies, evaluation and financing of Primary Care within the framework of the Unified Health System.
