RELIGIOUS-SPIRITUAL COPING STRATEGIES IN PATIENTS UNDERGOING HEMATOPOIETIC STEM CELL TRANSPLANTATION: A CROSS-SECTIONAL STUDY

ABSTRACT

Introduction: Coping is a mental construct that represents how each person faces a threatening and stress-inducing situation. When coping strategies involve religiosity and spirituality, religious-spiritual coping (RSC) is obtained. RSC is associated with the context of hematopoietic stem cell transplantation because transplant patients may present coping strategies that involve religiosity and spirituality; these strategies are related to the appearance of positive or negative outcomes that consequently impact treatment. Thus, this study aims to analyze RSC strategies in patients undergoing hematopoietic stem cell transplantation (HSCT). Method: This is a quantitative, cross-sectional, descriptive, and analytical study involving patients who underwent HSCT at a reference center in the state of Rio Grande do Norte (RN), Brazil, between May and October 2025. Two data collection instruments were used: a sociodemographic and clinical characterization questionnaire, and the abbreviated Religious-Spiritual Coping Scale. In compliance with Resolution 466/2012, which defines the guidelines and regulations for research involving human subjects, this project was submitted to the Research Ethics Committee of the Federal University of Rio Grande do Norte and initiated only after obtaining a favorable opinion. Results: The final sample consisted of 89 participants, 53.70% were male, 26.97% were in the 40-49 age range, 52.81% self-identified as mixed-race, 55.06% practiced the Catholic religion, 38.20% resided in municipalities in the interior of the state of RN, and 68.20% were married or in a stable union. Furthermore, 28.29% had acute myeloid leukemia as a diagnosis indicating the need for transplantation, 51.52% had systemic arterial hypertension, 58.43% underwent allogeneic transplantation, and 79.78% had hematopoietic progenitor cells collected from peripheral blood. The most frequently used positive RSC strategies relate to self-transformation and/or life transformation, offering help to others, a positive stance towards God, actions in seeking institutional support, personal pursuit of spiritual knowledge, and distancing oneself from God, religion, and spirituality. Strategies related to seeking spiritual help were not used at all by the majority of participants. Regarding the negative dimension, strategies related to a negative stance towards God were identified as very frequently used; the other strategies were not used at all by the majority of patients. It is noteworthy that the studied sample showed low use of negative coping (mean = 1.58), medium use of positive coping (mean = 2.98), and total coping (3.42). Furthermore, positive coping strategies were used more frequently than negative ones by the participants. Conclusion: Thus, the strategies used were prayers, developing a closer relationship with God or the Divinity through requests for help, forgiveness, and personal transformation. Likewise, offering comfort and wishing well to others, practicing acts of charity and engaging in volunteer activities, seeking support and protection from divinity, as well as strength and guidance, listening to and/or singing religious songs, seeking help in sacred texts, associating personal responsibilities with treatment with faith in surrendering one's life and fears to the supreme being in whom one believes.
