SUMMARY:

Annually, approximately 10 million individuals worldwide develop tuberculosis (TB). The incidence among incarcerated individuals is 26 times higher than in the general population. This disparity is driven by factors such as lack of systematic screening, overcrowding, and inadequate isolation facilities for sick individuals. Therefore, evaluating evidence-based TB control measures within the prison system is crucial for disease control in this context. Objective: To assess TB control measures in correctional facilities in the Metropolitan Region of Natal. Method: This evaluative research follows the framework proposed by Hartz and Silva (2005), employing a quantitative and descriptive approach. The theoretical foundation for the quantitative approach is based on Donabedian's normative evaluation perspective, focusing on TB control actions in correctional facilities through the Structure and Process dimensions. The study was conducted in correctional facilities across Natal, Parnamirim, Nísia Floresta, and Ceará-Mirim. The study population comprised health professionals from prison health teams and health surveillance managers in each municipality. Data collection utilized a questionnaire developed by researchers and technicians in the field of tuberculosis, referencing the Brazilian TB Control Manual, which provides elements for constructing indicators. Quantitative data were organized and processed using SPSS Statistics® version 22.0, presented through descriptive and inferential statistics, including mean values and the degree of implementation of TB control actions in the facilities. A logistic regression model was constructed to identify factors associated with the degree of implementation and their influence in the context. Qualitative data from interviews were processed using the Interface de R pour les Analyses Multidimensionnelles de Textes et de Questionnaires software, followed by content analysis as per Bardin's methodology. The study was approved by the Ethics Committee of the Federal University of Rio Grande do Norte under opinion No. 7.612.481, adhering to ethical principles outlined in Resolution No. 466/12 of the National Health Council. Results: The logical model supported the evaluation process and understanding of TB control actions in correctional facilities through the Structure- Process evaluation framework. Of the 60 professionals selected, 48 participated in the final sample. The majority were affiliated with the Dinorá Simas Lima Deodato Public Jail (25.0%) in Ceará-Mirim and the Alcaçuz State Penitentiary (22.92%) in Nísia Floresta, followed by the Parnamirim State Penitentiary (18.75%) and the Dr. João Chaves Penal Complex (12.50%) in Natal. A smaller proportion worked at the Parnamirim Provisional Detention Center (8.33%). The overall degree of implementation of TB infection control actions was classified as partially implemented (53.1%). The Structure and Process dimensions were rated as partially implemented (52.25%) and (55.06%), respectively. Within the Structure dimension, administrative measures were partially implemented (71.9%), environmental control measures were not implemented (8.33%), and respiratory protection measures were partially implemented (65.9%). In the Process dimension, administrative measures were partially implemented (70.42%). Factors associated with the degree of implementation identified through logistic regression included the use of the Respiratory Symptom Register Book, differential service hours for suspected cases, and the presence of a disease control plan. Conclusion: The development of the logical model for TB control actions highlighted both strengths and weaknesses in addressing the disease within correctional facilities, particularly in the Structure and Process dimensions. The degree of implementation in these areas was classified as partially implemented, indicating limited progress and significant gaps. Administrative and respiratory protection measures were only partially implemented, while environmental control measures remained unimplemented. This underscores the need to strengthen actions, especially regarding the comprehensive adoption of disease control measures, to ensure effective TB prevention and control strategies in the prison environment, involving all relevant stakeholders.
