SUMMARY:

Palliative extubation consists of removing devices used to maintain breathing in patients at the end of their lives when ventilatory therapy is no longer considered beneficial. This practice faces several barriers, such as cultural and religious influences on health professionals, patients and their families, as well as the lack of clear guidelines and protocols in intensive care units. These factors contribute to the adoption of varied clinical procedures and to the lack of knowledge about end-of-life care. The objective of this study is to develop and validate an educational technology on palliative extubation for health professionals. This is a mixed-methodological study based on the stages of Instructional Design. In the first stage, analysis, a scoping review and a descriptive-exploratory study were carried out to assess the knowledge and difficulties of health professionals on the subject. This phase was conducted in the intensive care units of two university hospitals in the Northeast and Southeast regions of Brazil, with the participation of nurses, physicians, physiotherapists and nursing technicians. Data collection was carried out through an electronic form sent to institutional e-mails. The data obtained were analyzed using statistics and thematic content analysis. The second stage, design and development, involved the authorship and creation of a base text, followed by scripting and preparation/editing of the material. In the evaluation stage, content and appearance validation was performed with specialists in the areas of nursing, medicine, and physiotherapy, recruited via the Lattes platform. The results of the review identified 44 studies, addressing guidelines, symptom management, time between extubation and death, institutional protocols, and emotional aspects of professionals. The exploratory research involved 73 health professionals, most of them female, with an average age of 38.6 years and 7.78 years of experience in critical units. Most had a specialization or master's degree. The average performance in the knowledge test was 8.89 correct answers and a significant association with the work institution (p = 0.007) with the best performance of professionals from the Southeast region. The main difficulties reported included lack of qualifications, absence of protocols, communication failures, family resistance and fear of legal implications. In the design and development stage, the e-book was developed based on evidence and with technical content on palliative extubation, symptom management, use of drugs and interactive resources. Finally, 13 expert judges participated in the evaluation of the e-book, including six nurses, four physicians and three physiotherapists, most of whom had doctorates. The average age of the evaluators was 42.54 years, with an average time since graduation of 19 years. The indexes obtained were significant: Content Validity Index (0.95), Kappa Coefficient (0.81) and Appearance Validity Index (0.97). It was concluded that the validated educational e-book represents a relevant contribution to the training of health professionals, by offering an accessible, well-founded and qualified tool on palliative extubation - a topic that is still sensitive and little explored in the daily routine of intensive care units.
