SUMMARY:

ABSTRACT

Introduction: The diaphragm is the primary respiratory muscle, and its dysfunction can occur in various pathological conditions, including coronary artery bypass grafting (CABG). The postoperative period of CABG is associated with pulmonary impairment, respiratory muscle weakness, and pain, potentially leading to reduced ventilatory capacity and an increased risk of pulmonary complications. Respiratory sarcopenia, characterized by the loss of respiratory muscle mass and strength, may be a relevant prognostic factor in the intensive care unit (ICU), being associated with reintubation rates and mortality. Understanding the prevalence and consequences of this condition in the immediate postoperative period of CABG may contribute to the development of more effective respiratory rehabilitation strategies and improve patient prognosis. Objectives: To analyze the prevalence and characteristics of respiratory sarcopenia in individuals in the immediate postoperative period of CABG. Specifically, to evaluate static and dynamic respiratory muscle strength through maximal inspiratory (PImax) and expiratory (PEmax) pressures, as well as the S-index. Additionally, to investigate the morphofunctional aspects of the diaphragm using kinesiological ultrasound, considering diaphragm thickness/thickening fraction (TF), diaphragmatic mobility (DM), and contraction velocity (CV), correlating these variables with clinical-functional indicators. Methods: This is an observational, cross-sectional study to be conducted in the ICU of the Onofre Lopes University Hospital (HUOL) in Natal, RN, between May 2025 and July 2026. The sample will consist of individuals aged 40 to 80 years who have undergone CABG, selected according to predefined inclusion and exclusion criteria. The research will comply with the ethical principles outlined in Resolution 466/12 of the Brazilian National Health Council (CNS), ensuring data confidentiality and participant autonomy. Expected Results: The findings of this study may contribute to a better understanding of respiratory sarcopenia in the cardiovascular surgical context, providing support for the implementation of more effective and early respiratory rehabilitation strategies, with a positive impact on functional recovery and quality of life in patients undergoing CABG.
