SUMMARY:

Introduction: This thesis aims to contribute to the improvement of clinical
management and treatment adherence for individuals with chronic respiratory
diseases (CRD) and post-COVID-19 syndrome by synthesizing scientific
evidence and validating assessment tools. Methods: The research comprises
four studies, each structured as a scientific article. The first presents the
methodological protocol of a systematic review and meta-analysis on the
effects of home healthcare for adults with CRD, published in the BMJ Open
Journal. The second study, conducted in accordance with the Preferred
Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA)
guidelines, assessed the effectiveness of home healthcare based on

outcomes such as mortality, length of hospital stay, quality of life, self-
efficacy, functionality, hospital readmissions, patient satisfaction, costs, and

adverse events. A total of 20 studies, including 2,613 participants, were
analyzed. The third study involved the translation and cross-cultural
adaptation of the PROMIS® Self-Efficacy for Managing Daily Activities item
bank, with contributions from experts and target populations in Brazil and
Portugal. The fourth study, currently under development, focuses on the
psychometric validation of this instrument in a multi-country context, aiming to
enhance its clinical and scientific applicability across Lusophone settings.
Expected results: The findings of this thesis will significantly improve the
delivery of home healthcare, particularly in public health systems, and

promote more efficient use of public resources. The systematic review and
meta-analysis are anticipated to provide new evidence on the effectiveness of
home-based care compared to hospital care, affecting key out-comes such as
mortality, hospitalization duration, readmissions, functionality, and quality of
life. Additionally, the culturally adapted PROMIS® instrument is expected to
offer a robust and sensitive tool for evaluating self-efficacy, supporting
personalized interventions, and promoting health education and patient
empowerment in chronic condition management.
