SUMMARY:

Introduction: Chronic shoulder pain is one of the leading causes of musculoskeletal pain worldwide, resulting in decreased functionality, fear of performing certain activities, and avoidance of these tasks. Individuals with this condition commonly present deficits in rotator cuff strength, significant reductions in shoulder range of motion and mobility. People with shoulder pain also tend to exhibit high levels of depressive symptoms, anxiety, pain catastrophizing, and poor sleep quality. The Shoulder Performance Activity Test (SPAT) was developed to assess upper-limb performance- based functionality. However, there is still a lack of studies evaluating the influence of individual, physical, psychological, and sleep-related factors on SPAT performance. Objectives: To identify the extent to which individual, physical, psychological, and sleep-related characteristics are associated with upper-limb performance based functionality. Methods: This will be a cross- sectional study including 158 individuals with shoulder pain, residents of Santa Cruz–RN and nearby towns, aged 18 to 64 years, with unilateral, atraumatic shoulder pain for at least 3 months and a minimum pain through movement score of 2 on the Numerical Rating Scale (NRS). Sociodemographic, anthropometric, and clinical data will be collected, as well as physical activity level. Participants will then complete the following self-report questionnaires: shoulder pain and function using the Penn Shoulder Score; pain self-efficacy using the Pain Self-Efficacy Questionnaire (PSEQ-10); pain catastrophizing using the Pain Catastrophizing Scale (PCS); avoidance of movement using the Avoidance of Daily Activities Photo Scale for Patients With Shoulder Pain (ADAP Shoulder Scale); anxiety and depression using the Hospital Anxiety and Depression Scale (HADS); and sleep quality using the Pittsburgh Sleep Quality Index (PSQI). Afterwards, participants will perform the SPAT using the painful upper limb. Finally, they will undergo assessments of muscle strength for shoulder lateral rotation, arm elevation and extension; mobility using the Functional Movement Screen – Shoulder Mobility Test; power using the Unilateral Shot Put Test; and coordination using the Fugl-Meyer Assessment for the Upper Extremity (FMA-UE). Expected results: The study is expected to examine the influence of individual, physical, psychological, and sleep-related factors on upper-limb functional performance.
