SUMMARY:

This dissertation problematizes the social construction of madness regarding male subjects in sensitive institutional settings, focusing on the Centro de Atenção Psicossocial III (Psychosocial Care Center III) Tozinho Gadelha, located in Sousa, Paraíba. It investigates how gender representations and the constraints of masculinity shaped the life stories and experiences of mental illness among the institution's day-users between 2003 and 2024. The city of Sousa, characterized as a geopolitical hub and a pioneering center in the Alto Sertão region of Paraíba, established the region's first Type III alternative care service, whose institutional data reveal disparities and asymmetries in the attendance rates of men and women. This study’s general objective consists in investigating the social construction of madness surrounding the male day-users of the Tozinho Gadelha CAPS III in Sousa, Paraíba, aiming to understand how these individuals are represented and signified within discourses on mental suffering in the Alto Sertão context. The theoretical framework draws upon the reflections of Juciene Andrade (2020) to conceive the sertões (hinterlands) as dynamic, political spaces constructed from multiple material and symbolic imaginaries; Michel Foucault (1978) to discuss madness as a phenomenon conditioned by the knowledge-power dynamics of specific contexts; and Durval Muniz de Albuquerque Júnior (2013) to analyze the gender mechanisms that forged the normative ideal of virility and the myth of the “cabra-macho” (the quintessential “tough man”) in the Northeast region. Methodologically, the historiographical operation employs a tripartite set of sources: document analysis of the CAPS III implementation project; interviews based on Oral History with key figures in the local Psychiatric Reform; and a sociodemographic analysis of the day-users' medical records. The documentary corpus is examined through the lens of Michel Foucault’s A Ordem do Discurso (The Order of Discourse, 2004), cross-referencing sociodemographic data with the subjects' own narratives. The study concludes that the cultural imperative of male invulnerability acts as a powerful, invisible barrier to institutional care; it thus contributes to enriching the fields of the History of Health, Cultural History, and Gender Studies by bringing to light the stories of the men who embody madness in the sertões of Paraíba.
