Efficacy of Desensitizing Toothpastes on Tooth Sensitivity During At-Home Bleaching with Hydrogen Peroxide: A Randomized Clinical Trial

This study evaluated the efficacy of different desensitizing toothpastes in controlling tooth sensitivity during supervised at-home tooth whitening with 7.5% and 10% hydrogen peroxide (HP), as well as their influence on whitening efficacy, gingival condition, and oral health-related quality of life. A randomized, triple-blind clinical trial was conducted with 60 participants distributed into six experimental groups (n = 10), based on the whitening agent concentration and the type of toothpaste used (conventional, neural-action desensitizing, and tubule-occluding desensitizing). Assessments were performed at baseline and after 15 days of treatment using the Visual Analog Scale for tooth sensitivity, digital spectrophotometry for color analysis (ΔE00), the Gingival Bleeding Index, and the Oral Health Impact Profile (OHIP-14) questionnaire for quality of life. All protocols produced clinically relevant color changes, with no statistically significant differences in whitening efficacy among the groups. Although tooth sensitivity increased in all groups, scores remained low; however, they were significantly higher in the group treated with 10% HP combined with conventional toothpaste compared to the groups using 7.5% HP combined with neural-action (p = 0.043) and tubule-occluding (p = 0.009) desensitizing toothpastes. No differences regarding gingival condition were observed among the groups. Regarding oral health-related quality of life, a significant reduction in the total OHIP-14 score was observed in the groups using 7.5% HP + neural-action desensitizing toothpaste (p = 0.034), 10% HP + neural-action desensitizing toothpaste (p = 0.011), and 10% HP + occluding-action desensitizing toothpaste (p = 0.017), with no differences between the groups (p = 0.846). It is concluded that all protocols showed similar whitening efficacy and did not adversely affect gingival health. Although an increase in tooth sensitivity occurred in all groups, the highest pain levels were observed with 10% HP combined with conventional toothpaste. Furthermore, the use of desensitizing toothpastes was associated with improved oral health-related quality of life.
