SUMMARY:

Introduction: The C-expander is a tissue-bone-supported appliance that anchors to the maxilla through miniscrews installed obliquely in the interradicular spaces of the upper teeth. However, there is no consensus on the ideal regions for the placement of the miniscrews for this appliance. Objective: To evaluate interradicular distance, alveolar bone thickness, palatal cortical bone thickness, and mucosal thickness in the posterior alveolar region of the maxilla using cone-beam computed tomography (CBCT), with the aim of suggesting the ideal regions for the placement of mini-implants for the C-expander appliance. Material and Methods: The sample will consist of 160 CBCT scans of the maxilla selected from the database of a Dental Radiology center. CBCT scans of the maxilla from individuals of both sexes, aged 15 years or older, will be included. The sample will be divided into 5 age groups: the young group, which will include individuals aged 15 to 19 years; the adult group, including participants aged 20 to 39 years; the mature adult group, with individuals aged 40 to 59 years; and the elderly group, with participants aged 60 years and older. The RadiAnt DICOM Viewer® software will initially be used to standardize the position of the maxilla. Next, the measurements tool will be used to measure the smallest interradicular distance distal to the canines, first premolars, second premolars, first molars, and second molars, in axial slices located 4, 6, 8, 10, and 12 mm from the gingival margin (GM). In the axial slices and at the same levels, bone thickness will also be measured up to the minimum interradicular distance of 1.5 mm, representing the diameter of the interradicular miniscrew. Alveolar bone thickness, palatal cortical bone thickness, and palatal mucosa thickness will be measured in coronal slices at angles of 30°, 45°, 60°, and 75° relative to the occlusal plane, at the same interradicular sites and vertical levels of distance from the GM. These measurements will be performed on one side of the maxilla, right or left, which will be selected randomly. The Kolmogorov-Smirnov test will be used to assess the distribution pattern of the studied variables. If the variables show a normal distribution, the mean and standard deviation will be used to describe the participants’ age, while relative and absolute frequencies will be used to assess the distribution of the sample's sex. The mean, standard deviation, minimum, quartile 25%, median, quartile 75%, maximum and 95% confidence interval will be used to describe the mucosal thickness and the thickness and width of the interradicular spaces. Intergroup comparisons based on sex will be conducted using the student’s t-test, while comparisons between different age groups will be performed through Multivariate Analysis of Variance (MANOVA), followed by post-hoc multiple comparison tests if statistically significant differences are observed. Finally, multiple linear regression will be used to determine the relationship between sex and age with the measured data. A significance level of 5% will be adopted for all comparisons.
