SUMMARY:

Introduction: Currently, the use of dental implants to replace lost teeth and satisfy functional and aesthetic needs is one of the primary dental treatments. As prosthetic abutment options have evolved, patients' aesthetic expectations have concomitantly increased. Objective: This study aims to evaluate and compare the aesthetic outcomes, patient satisfaction, quality of life, and clinical performance of single crowns supported by implants using either prefabricated or hybrid abutments in the anterior maxillary region. Materials and methods: A controlled, double-blind, longitudinal clinical trial was conducted at the Federal University of Rio Grande do Norte (UFRN), involving 30 implants placed in the anterior maxillary region (incisors, canines, and first premolars). Following random allocation by blocks, customized hybrid titanium and zirconia abutments (test) or prefabricated titanium abutments (control) were installed. All patients underwent a tissue conditioning phase with a temporary crown before the final crown installation. Radiographic and photographic records, as well as clinical information regarding failures and complications, were collected. Aesthetic outcomes were evaluated using the PES (Pink Esthetic Score) and WES (White Esthetic Score) indices, satisfaction was assessed using the visual analogue scale (VAS), and quality of life was measured with the Oral Impacts on Daily Performances (OIDP) and the Oral Health Impact Profile (OHIP-14). The clinical survival rate was determined using modified CDA/Ryge criteria. Follow-up assessments were conducted at baseline (T0), temporary crown installation (T1), final crown installation (T2), and at 1 month (T3), 3 months (T4), 6 months (T5), 1 year (T6), and 2 years (T7) post-cementation. The null hypothesis posits that there is no statistically significant difference between the two groups. The split-plot ANOVA test will be used to analyze satisfaction, OHIP-14, and OIDP data. The Ryge criteria will be qualitatively evaluated, and restoration survival will be assessed using Kaplan-Meier and Log-Rank tests. Statistical analyses will be conducted with a significance level of 5%. Preliminary results: The treatment significantly improved quality of life, particularly when comparing analyses at T2 with T0, regardless of the group, with a statistically significant difference between the groups only at T2 (p<.005). Both groups achieved an A classification for the integrity and marginal discoloration criteria. However, a C classification for restoration integrity was observed in the test group.
