IMPACT OF NEW COMPLETE DENTURE REHABILITATION ON THE FACIAL PARAMETERS OF EDENTULOUS PATIENTS: A 3D STEREOPHOTOGRAMMETRY STUDY

SUMMARY:

This clinical trial aimed to evaluate the changes in soft tissue facial parameters in completely edentulous patients before and after rehabilitation with new prostheses, using three-dimensional stereophotogrammetric image analysis of the patient's face over a one-year follow-up period. A total of 28 edentulous patients were rehabilitated with new complete dentures. Calibrated and stereoscopic images of the patient's face were captured without prostheses (WP), with old prostheses (OP), and with new prostheses (NP) using a passive stereoscopic digital camera LifeViz Mini (Nikon D5600 body, Dermapix software, Quantificare S.A., 06410 Biot, France). The images were then analyzed using the Quantificare 3D Viewer software (Quantificare S.A., 06410 Biot, France) at the time of prosthesis installation. Image capture and analysis were also performed after one year in the edentulous condition (WP-1) and with the new prostheses (NP-1). Patient perception of their facial appearance before and after the installation of new prostheses was measured using the adapted Orofacial Esthetic Scale (OES) questionnaire. Patients were asked if they were interested in undergoing facial harmonization procedures to improve their facial appearance with old and new prostheses. Data normality was determined using the Shapiro-Wilk test. Paired T-test or Wilcoxon test was applied for linear and angular evaluations of facial parameters, and patient perception of facial appearance was analyzed using the Wilcoxon test. The Chi-square test was used to verify the association between patients' interest in facial harmonization procedures with old and new prostheses. Statistical tests were conducted considering a significance level of 5% and a test power of 80%. The majority of the study sample consisted of women (n=21; 75%). The mean age of the patients was 65.89±8.61 years, and the mean duration of edentulism in the maxilla and mandible was 27.96±13.85 years and 26.68±14.81 years, respectively. Facial parameters between WP-OP showed significant differences in 7 out of 13 linear measurements, while comparisons between OP-NP and WP-NP evidenced 10 and 11 changes in the facial lengths of the middle and lower thirds of the face, respectively. The absence of prostheses resulted in a significant retraction of the upper (Ls-(Prn-Pg)) and lower lips (Li-(Prn-Pg)) relative to the facial profile, contributing to the collapse of the middle and lower thirds of the face (p<0.001). For angular measurements, rehabilitation with new prostheses significantly improved facial parameters compared to the edentulous condition and old prostheses, contributing to an increase in the convexity of the patient's facial profile [G-Sn-Pg, p<0.05)]. For comparisons between baseline (WP) and one-year follow-up in the edentulous condition (WP-1), the median height of the upper portion of the lower third (N-St) significantly increased (p=0.020), while the mean angle of the lower lip decreased (p=0.001). Regarding measurements with new prostheses between the installation time (NP) and one year of use (NP-1), 2 measurements decreased (Prn-St and N-Prn-Cm) and 3 measurements increased (Ls-(Prn-Pg)), (Li-(Prn-Pg)), and Li-Sm-Pg), with these differences being significant (p<0.05). Patient perception of facial appearance was satisfactory with the use of prostheses compared to the edentulous status, with these results being more pronounced after rehabilitation with new prostheses (p<0.001). These findings were maintained after one year, despite no significant difference being evidenced (p>0.05). Although no significant association was found, interest in undergoing facial harmonization procedures decreased by 53% after rehabilitation with new prostheses (p=0.226) and increased by 23% for the one-year follow-up analysis (p=0.744). In conclusion, edentulism and old prostheses with prolonged use alter the facial parameters of the middle-lower third of the face. Rehabilitation with new prostheses can restore tissue support and alter the proportions of the facial profile of completely edentulous patients, reducing the collapse of the tissues in the lower third of facial aesthetics. Soft tissue facial parameters after rehabilitation with new prostheses were maintained after one year of follow-up, except for the superior facial height and labial projection and nasolabial angle, which decreased, and the lower labial projection and angle, which significantly increased. Patient perception of facial appearance was significantly satisfactory when comparing the edentulous condition and/or old prostheses with the use of new prostheses, and these results were maintained after one year of follow-up. Rehabilitation with new prostheses did not influence the interest in facial harmonization, indicating that conventional prostheses improve facial aesthetics and meet patient expectations regarding the harmonious appearance of the facial profile.
