Hospital efficiency in Brazil comes up against the incipience of care management, the excess of bureaucracy, the lack of standardization of the process and the unavailability of practices that promote the quality of care efficiency. This project aims to design a care process management model capable of generating quality and care efficiency, focusing on the patient. There is an opportunity to create value for patient care during hospitalization, impacting performance indicators in healthcare, especially during hospitalization. Furthermore, there is a gap in the knowledge of the care team and a gap in tools that promote a standardized and participatory care management. Therefore, this project will be aimed at the exhibition of two products: (1) the promotion of lean education as a risk management strategy for implementing lean projects in the care environment; (2) a method of designing the care processes of the care line, using lean six sigma, as a way to promote the effective management of the patient's journey during a hospital stay. The products were designed using the DMADV (Define, Measure, Analyze, Design and Verify) methodology of the For Lean Six Sigma design. In the concept of Health 4.0, when developing a care process, it is expected that an innovative practice promotes differentiated care management, which can be structured by lean six sigma. As a result, a reduction in errors is expected with an improvement in quality, a decrease in costs and the leverage of care performance
