EDUCATIONAL AND CARE TECHNOLOGY: CONSTRUCTION AND VALIDATION OF AN INDIVIDUALIZED PLAN AS A STRATEGY FOR QUALIFYING SPECIALIZED AMBULATORY CARE AND TEACHING-SERVICE INTEGRATION.

Addressing chronic degenerative conditions is a critical challenge for public health in Brazil, requiring a holistic and coordinated approach. Care for chronic degenerative conditions is a priority in the Brazilian Unified Health System (SUS) due to the high demand for care and the need for early interventions to prevent complications. The objective of this study was to develop and validate an Individualized Care Plan, configured as educational and care technology to strengthen teaching-service integration. The instrument was developed based on a narrative review of the literature, which identified essential components for the care of people with chronic conditions. The validation process took place in two complementary stages. Content validation was performed by professionals and preceptors working at the Seridó Regional Polyclinic, by PHC professionals and preceptors, and by PHC residents in the municipality of Caicó (RN), using the Content Validation Index (CVI). Next, appearance validation was conducted with the same group of professionals and judges from the field of computing, using the Suitability Assessment of Materials (SAM) tool. In both stages, the technical product achieved scores above the minimum required, demonstrating high acceptability and confirming the relevance, clarity, and importance of the items that comprise the instrument. It is concluded that the validated Care Plan has the potential to improve the clinical and educational management of patients with chronic conditions, promoting greater integration between levels of care and strengthening collaborative practices in the territory. It is a technology with the capacity to contribute to improving the care experience, optimizing the work of teams, and, consequently, to the efficiency and sustainability of the health system as a whole.
