EVALUATION OF THE SLEEP/WAKE CYCLE, CAREGIVER BURDEN AND LEVEL OF FUNCTIONALITY OF THE ELDERLY: A MIXED METHODS STUDY

Introduction: The act of caring for an elderly person involves daily changes, responsibilities, tasks and availability of time, as well as negative implications for the carer's health, which can affect their quality of life and their sleep/wake pattern. Objective: to assess the self-perception and the association of the sleep/wake cycle and quality of the caregiver's sleep with the level of workload and functionality of the elderly. Methodology: study of mixed methods, of the sequential type, in which quantitative research was carried out, with transversal, analytical and exploratory delineation; and qualitative research oriented through the technique of content analysis in the thematic modality. 73 elderly caregivers linked to the Family Health Strategy from a Paraiban municipality, randomly selected, participated in the survey. The data were collected between April and August 2023 through eight instruments, including: sociodemographic and occupational questionnaire; World Health Organization Disability Assessment Schedule 2.0; Zarit Burden Interview; Pittsburgh Sleep Quality Index; sleep diary, Maldonado Sleep Scale and a semi-structured roadmap for the in-depth interview. The quantitative data were analyzed from descriptive and bivariate statistics, considering significance when p-value < 0.05. The qualitative data were processed by the IRAMUTEQ software with the subsidy of the descending hierarchical classification, and subsequently analyzed in the light of the Bardin methodological reference. All the procedures carried out were guided by Resolution No. 466/2012 of the National Health Council. Preliminary results: 47.9% of the total sample showed poor sleep quality, which was associated with the female sex (x2=5.58p-value = 0.018), Urban Housing Area (x2=15.89p- Value = 0,0001) and Household Occupation (X2=11.56p-valu = 0.0001). When considering the stratifications of ZARIT, 67.1% of caregivers present a moderate to severe degree of overload associated with the poor quality of the caregiver's sleep (p-value < 0,0001), the correlation of which is positive (ρ = 0.727) indicating that the greater the degree of supercharge of the Caregiver, the worse will be their sleep quality. The higher the degree of dysfunctionality of the elderly, the greater the caregiver's burden (ρ = 0.794; p-value < 0,0001). By comparing bedtime, rise and bedtime between weekdays and weekends, it was found that bedtime and rise times are later on weekends (p-value <0,0001), with no difference in bedtime. The qualitative data corroborate the findings of the quantitative set, these being presented in two thematic categories, which reveal the caregiver's perception under the importance of sleep, the dissatisfaction of the quality and the habits necessary for its proper maintenance. Discourses have also emerged that characterize care activity as compromising the sleep cycle and the other biopsychosocial dimensions of the individual.
