Introduction: During pregnancy, there is an intense anabolic activity for ensure adequate fetal development. Thus, hemoglobin levels are expected to be reduced from the first to third trimester of pregnancy and return to pre-pregnancy levels on the post-partum. However, little is known about this variation when comparing adolescent and adult pregnant women and what factors are associated with this variation. Objective: The aim this study was evaluate the trajectories of total hemoglobin (THB) variation during pregnancy and postpartum and its associated factors among low-income adolescents and adults. Methods: This is an observational, longitudinal study that is part of the pilot project AMOR. The data was collected between July 2017 and May 2018 in five municipalities of the Trairi region of the Rio Grande do Norte State, with a sample consisted of 50 pregnant adolescents and 50 adults who were in their first pregnancy. The participants were evaluated in three moments: up to 16 weeks of pregnancy, in the third trimester of pregnancy and between 4 and 6 weeks postpartum. Variables as socioeconomic characteristics, iron supplementation, self-perceived health, number of prenatal appointments, planned pregnancy and food security were evaluated. Mixed linear models were used to evaluate the trajectories of THB in both groups and in the total sample, and the associated factors. Results: The results show that THB levels decrease from the first 16-weeks of pregnancy to the third trimester and increase from the third trimester to the 4-6 weeks postpartum for both groups. However, both groups differ in relation to the THB trajectories. For the adolescent cohort, the rebound in THB concentration between the third trimester and postpartum was not enough to make up for the initial losses, as happened for the adult cohort. For the adolescent group, higher THB levels were associated to good self-rated health. For the adults, higher THB levels were associated with planning of pregnancy. Race was marginally associated to THB levels for both groups, with black women with higher concentrations. Conclusion: Changes in THB concentration during pregnancy and postpartum differ between adults and adolescents, with the latter presenting lower capacity to recover levels from the beginning of pregnancy during postpartum. Prenatal care during adolescent pregnancy should consider the higher risk of this group and include strategies that reduce the risk of anemia and its negative effects for the mother and her child.
