SUMMARY:

Introduction: Neuromuscular alterations are observed with aging, including body composition and muscle strength. Reductions in appendicular muscle mass (AMM) and muscle strength are associated with adverse health-related outcomes in older adults. However, the relationship between AMM and upper and lower limbs muscle strengh with quality of life (QoL) is poorly understood in older adults. Objectives: To investigate the association between AMM and upper and lower limbs muscle strength with QoL in older adults. Methods: Cross-sectional study including 399 community-dwelling older adults aged 60-80 years. AMM was assessed by bioelectrical impedance. Muscle strength was assessed by handgrip strength (HGS; upper limbs) and 30 s chair stand test (CST; lower limbs). Results below 25th percentile for AMM, HGS and CST, stratified by sex, were considerated as “low AMM” and “low muscle strengh”, respectively. QoL was assessedby WHOQOL-BREF questionnaire (general QoL and domain-specific QoL: physical, psychological, social, environmental). General and domain-specific QoL scores classified as “needs to improve” and “regular” were grouped for multivariate analysis. Results: There was a positive correlation between AMM with pshycological and social domains of QoL (r = 0.105-0.121; p < 0.05). No significant correlation was observed between HGS and QoL (p > 0.05). There was a positive correlation between CST with general QoL and all domain-specific QoL (r = 0.175-0.343; p < 0.05). Low AMM was associated with low psychological domain of QoL (prevalence ratio [PR]: 1.30, IC 95% 1.12–1.52). Low muscle strength by HGS was associated with low general QoL (PR: 1.09, IC 95% 1.01–1.17). Low muscle strength by CST was associated with low general QoL (PR: 1.20, IC 95% 1.05–1.39), physical (PR: 1.34, IC 95% 1.10–1.63), psychological (PR: 1.34, IC 95% 1.10–1.63), and environmental (PR: 1.12, IC 95% 1.00–1.24) domains of QoL. Conclusion: Muscle strength, mainly from lower limbs, is associated with QoL in community-dwelling older adults, which does not occur with AMM.
