SUMMARY:

Variation in blood glucose levels is a common complication during hemodialysis and is related to the pathophysiology of kidney disease and hemodynamic changes resulting from cardiopulmonary bypass. Thus, the nursing diagnosis risk of unstable blood glucose can be inferred in this clientele, however, the NANDA International taxonomy does not identify hemodialysis patients as a population at risk. That said, the present study aims to validate the nursing diagnosis of risk of unstable blood glucose, based on a medium-range theory, in patients undergoing hemodialysis. This is a methodological study of validating a nursing diagnosis, which will follow three steps proposed in the literature, namely: 1) construction of a medium-range theory of the aforementioned nursing diagnosis; 2) content analysis by experts and; 3) clinical causal validation. The first stage was operationalized through a Scoping Review guided by the JBI Reviewer's Manual and guided according to the recommendations of the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews: Checklist and Explanation, with a research protocol registered in the Open Science Framework: DOI 10.17605/OSF.IO/EGZFS. The second stage will be validation by experts, who will judge the suitability of the content created in the first stage; and, finally, in the third stage, the elements of the aforementioned nursing diagnosis will be clinically validated, through a case-control study. The data will be analyzed using descriptive and inferential statistics. The study was approved by the Research Ethics Committee, nº 5.906.314, CAAE: 67211023.7.0000.5537. In the first stage of the study, research that presented clinical indicators and causal factors of the nursing diagnosis of risk of unstable blood glucose in renal patients on hemodialysis was investigated. In May 2023, a search was carried out in the white and gray literature and the data was analyzed using simple descriptive statistics. The final sample consisted of 19 studies, published between 2006 and 2022, mostly in English and from 11 countries. In this way, from the articles selected by the Scoping Review, it was possible to list the essential attributes and clinical antecedents of the risk of unstable blood glucose, as well as to elaborate its conceptual and operational definitions in order to clarify its clinical meaning. In this way, it is expected to contribute to the accurate identification and greater clarity of the elements of this diagnosis in the clinical practice of nurses with patients undergoing hemodialysis, contributing to the advancement of nursing science.
