SUMMARY:

Users with difficult-to-heal wounds include those with wounds that have not healed 40-50% of their length after four weeks of analysis and appropriate clinical intervention. This problem is related to biological, clinical, non-clinical and service delivery factors, in addition, it requires the use of multiple services and levels of complexity of the health care network. Thus, Transitional Care becomes indispensable, a care modality that seeks to improve the results associated with users, their families, caregivers and health systems, during the transfer between different units, services and levels of care. Thus, the present study aims to analyze the management of Transitional Care for users with wounds that are difficult to heal, carried out by nurses and workers inserted in the health care network. This is a single integrated case study of a qualitative nature. Its unit of analysis is the health care network in the city of Natal-RN. It will be conducted according to the guidelines of Yin (2015). The following sources of evidence will be used: 1- documentary research in medical records for the clinical and socio-
epidemiological survey of users with wounds that are difficult to heal. 2- Semi-structured interviews of users with wounds that are difficult to heal were discharged from the hospital, as well as nurses and workers from the services that make up the health care network; 3- Scope review on the performance of the Nurse and health workers involved in the management of Transitional Care for users with wounds that are difficult to heal. Documentary data will be analyzed using the Statistical Package for the Social Sciences application, through descriptive statistics and multivariate analysis with Poisson regression. Data from semi-structured interviews will be analyzed using the hermeneutic-dialectic method proposed by Minayo (2006), supported by the ATLAS TI® software. Finally, the application of the “construction of explanation” technique will be applied, which will allow the triangulation of the collected data. The results will be presented using texts, figures, tables, charts and graphs, organized in the case study report. The research will meet the ethical precepts presented in Resolution nº 466, of December 12, 2012, of the National Health Council.
