SUMMARY:

The Intensive Care Unit is an environment specialized in providing care to critically ill patients, characterized by assistance of greater complexity. In this sector, patients are vulnerable to numerous procedures for maintaining life. Most of them are submitted to invasive ventilatory support through orotracheal intubation, and consequently, to the use of a support for nutritional therapy, since, temporarily, they are with impaired swallowing and unable to receive food in a physiological way. In this way, nasoenteral tube feeding is provided to satisfy nutritional requirements. Despite being a common procedure and despite the simplicity of the procedure, insertion of the tube is not without risks and can compromise patient safety, being one of the procedures most commonly performed by nurses. Traditionally, the probe is inserted ‘‘blind’’, with the patient’s head in a neutral position. Therefore, the success of the procedure also requires the patient's collaboration, which means that the nurse usually has greater difficulty to perform it in patients with altered level of consciousness, as it often occurs among critically ill patients, which can make difficult and traumatic. Thus, the important role of nursing in the research process on the effectiveness of these interventions is perceived, allowing to compare the new insertion techniques and the conventional method and to evaluate their effectiveness, considering the importance of the best practices aiming at the safety of the patient. Therefore, the objective is to evaluate the effectiveness of different techniques for inserting the nasoenteral tube in comatose (physiological or induced) and intubated patients. This is a prospective, randomized, controlled and blinded clinical trial to be performed on patients in a coma (physiological or induced) and intubated admitted to the Intensive Care Unit of the Dr. José Pedro Bezerra Hospital. Patients will be randomly assigned to three groups: those who will have the probe inserted in a conventional manner with the head in the neutral position, the group with the head positioned laterally to the right, and, finally, the group with the head in the neutral position, with assistance of a laryngoscope. The following will be assessed: (1) First, second and total success rate; Time required for the first successful attempt and the sum of all attempts; (3) Complications during insertion, including tube folding, twisting, knot, mucosal bleeding and insertion into the trachea; (4) Patient's vital signs. The collected data will be inserted in a database and exported to a computerized software, the Statistical Package for Social Science Version 20.0, where descriptive and inferential analyzes will be performed. The research project will be evaluated by the Research Ethics Committee of the Federal University of Rio Grande do Norte.
