SUMMARY:

The objective was to develop the concept of Patient Expert in the context of people with chronic non-communicable diseases. It is a methodological study, conducted by the Hybrid Concept Development Model proposed by Schwartz-Barcott and Kim. The study was developed in two stages, the first stage being a context analysis and the second stage a concept analysis. For the first stage, the four interactive levels of context proposed by Hinds, Chaves and Cypress were used, and were delimited as follows: Health care centered on the patient as a meta-context; Self-management of health care and the Expert Patient as a general context; Implications and challenges for the health care model as a specific context; and Particularities of the Expert Patient and interprofessional care as an immediate context. The second stage was made operational by a scoping review, using the Joanna Briggs Institute methodological framework, which resulted in a final sample of 145 studies. As a result of the concept analysis stage, of the total of studies analyzed, it was observed that the publications followed an increasing pattern in the 10-year period, where the majority occurred between 2015 and 2020. In addition, half of these studies were developed in the United Kingdom and 119 were published in the English language. Regarding the origin of the concept, it was found that it results mainly from the increase in chronic health conditions. Five expressions were found in the literature to describe the concept, the most cited being “Expert Patient” followed by “Specialist Patient”. In its practical and population aspects, the concept is predominantly related to family health units and people with diabetes mellitus. As for the essential elements of the concept, the search highlighted the presence of chronic diseases or comorbidities as the most frequent antecedent, health education for self-care as a primary attribute and independence in carrying out activities of daily living as the main consequence. Through the contextual analysis carried out added to the theoretical delimitation of the definitions, the practical, contextual aspects and the essential elements previously described, it was possible to develop the concept of Expert Patient in the context of people with chronic non-communicable diseases in a clear and precise manner, in agreement the Brazilian context.
