RESUMO:

Introduction: The current health profile of the Brazilian population, due to the demographic and epidemiological transition in the country, has demonstrated the growth of chronic diseases that cause health problems and threaten life. They are health conditions that cause discomfort in the physical, psychic, social and spiritual areas of the individual, directly interfering in their quality of life. This impact evidences the need for palliative care, which consists of the health care of people with chronic illness with quality of life as a priority. Adding palliative care to nursing care in the context of home care allows us to offer more comfort to the patient and his family. Considering the absence of a theoretical and methodological approach to orienting this nursing care to adult patients with chronic illness at home, this study addresses the following research question: What is the nursing model in palliative care for adults with chronic diseases elaborated from the actions of nurses in Home Care? Objective: To elaborate and validate a nursing model in adult palliative care in the context of Home Care. Method: It is a research, of the convergent kind, with a qualitative approach. It will be performed at the Home Care Service, in the city of Natal, capital of Rio Grande do Norte, Brazil. Participants will be professionals who integrate the multiprofessional team of home care and professionals of the support team. And by nurses of Family Health Strategies that are part of the coverage of said Home Care Service. Participants will be selected for convenience, and invited to participate in the study in accordance with the ethical principles necessary for the development of the research. The study will be developed in four steps: 1) Visit to the workplace of health professionals to survey the needs of health professionals and nurses in the provision of palliative care at home through participatory observation; 2) Meeting with the participating professionals, in order to elaborate a proposal of a theoretical model with methodological strategies; 3) Validation of the elaborated model, together with nursing professionals; 4) Return to the home of the participants in palliative care, in order to implement the strategies contained in the model in order to verify the results achieved. Data analysis in convergent care research, due to the versatile nature of this type of study, which covers assistance and research processes, combined methods in the collection and analysis of information may be used. Expected Results: it is hoped to identify the assistance needs of the professionals involved in the study, whether they are potential to the fragilities of palliative care. In addition, it is expected to build a nursing care model for home-based palliative care that can be implemented as a technological innovation to improve patients' quality of life. It is intended to contribute to the improvement of nursing practice.
