Validation of Nurse Competence Scale (NCS) to Brazilian Portuguese

The professional training and its process require the development of the competences necessary for the professional´s performance, although, it is common to leave some doubts regarding the competences of the newly graduated nurse. It is believed that this occurs due to the operationalization of the competence concept to be considered a complex task and hindering the construction and validation of evaluation instruments. In spite of this, some instruments for assessing the general competences of nurses have been elaborated and validated, most of them coming from the international literature, such as the Nurse Competence Scale (NCS). It is a self-report scale that evaluates the competences of the nurse, consisting of 73 items, distributed in seven categories, and it shows to be sensitive in the differentiation of nurses´ competences levels in different clinical settings and in work experiences, including the stage transition from nursing student to the nurse phase. This study aims to validate the Nurse Competence Scale to brazilian portuguese. This is a methodological type and hybrid approach study, developed after the authorization of the main author of the instrument and the approval by the Research Ethics Committee of the Federal University of Rio Grande does Norte, under opinion nº 1,766,255. Data collection was between september 2016 and june 2017, in two stages. The first stage has the cultural adaptation of the instrument and included the stages recommended by the international literature: translation, synthesis of translation, back-translation, review by the experts committee, pre-test of the adapted version and submission and evaluation of the reports by the author of the instrument. The second stage consisted of the evaluation of the psychometric qualities of the adapted instrument, applied in a sample of 111 undergraduate students of the Nursing Undergraduate course of two Public Institutions of Higher Education in the state of Rio Grande do Norte. For analysis, besides to content validity, there was construct validity, performed by confirmatory factorial analysis (CFA). Regarding the reliability measures, the Student's t-test was used to verify the stability of the scale through the test-retest; and the Cronbach's alpha coefficient was used to verify the internal consistency of the items. In the first stage, all the steps recommended for cultural adaptation occurred satisfactorily. The content validity index (CVI) for the instrument adapted as the Nursing Competency Scale (NCS) was 0.99, and the Kappa coefficient was considered almost perfect. Only 13 of the 73 items on the scale presented a significant difference in the test of comparison of means during the test-retest (n=30) for the competence level. The total Cronbach's alpha was 0.97, and in the categories ranged from 0.75 to 0.92. In the second stage, 111 undergraduate students participated in the Nursing Undergraduate course, with a mean age of 24.2 (SD=3.5); 84.7% of the female gender and 95.5% with no previous experience in the nursing area. The instrument was considered easy to fill, with an average application time of 15 minutes, in both steps. The students were self-assessed as having a good level of competences (average of 73.9), with an average of effective use of the items in practice, equal to 2, which corresponds to “used occasionally.” Regarding the categories, the general competence level was higher for the Supporting Function (80.5), and lower, for the Therapeutic Interventions category (67.8). Regarding the items, 68 (93.2%) were evaluated as good (> 50 - 75) and very good (> 75 - 100) for the competence level. In the analysis of the correlation between the items of the scale, it was moderate to strong, considering the categories among themselves. With emphasis on the correlation between Therapeutic Interventions with Educational Functions (0.71), Diagnostic Functions (0.74) and Management Functions (0.78); (0.74), with Therapeutic Interventions (0.79) and Quality Assurance (0.71), which presented a strong correlation between them. Regarding the construct validity, the structural equations model was used for latent variables. The adjusted model and its indexes found for the competence level were: c2 = 6.107,702; g.l. = 2.628; c2/g.l. = 2,4103; CFI   0, 411; TLI = 0,389; RMSEA = 0,113 e SRMR = 0,122 and the complexity of the model and the number of parameters estimated were considered, indicating a model fit satisfactorily for the sample studied. Most of the 73 variables (43) presented a very expressive variable-factor correlation, with factorial loads above 0.5. At this stage, Cronbach's alpha coefficient values were higher than 0.70, except for the Functional Support category, both for the competence level (0.65) and for the frequency of effective use of items in the clinical practice (0.67). It was concluded that the NCS validation process was reached, and the brazilian portuguese version called Escala de Competência do Enfermeiro (ECE) was valid, reliable and easy to apply in the sample studied. It is believed that ECE can be used as a tool to evaluate the competence of nursing undergraduates, although the regionality of the data is a limiting aspect of this study. Therefore, the performance of new studies are suggested to allow a deeper analysis of the psychometric qualities of the scale in other groups of Brazilian nursing students, in different contexts to allow comparisons between research results on the use of the NCS Brazilian version.
