RESUMO:

The systematization of Nursing Assistance (SAE) emerges as a methodology for the organization of nursing work, facilitates the customer care and provides scientific to the profession, as it requires the use of theories. The Theory of Basic Human Needs, Wanda de Aguiar Horta, consolidated the emergence of the Nursing Process (PE) in Brazil, a key tool for achieving the SAE, which gives formal aspects to nursing care. Currently, Parliament is regulated by Resolution 358/09, which deals with the SAE and its implementation. It is organized into five interdependent and recurring steps, namely: data collection, nursing diagnosis (DE), nursing planning, implementation and evaluation. The nursing diagnoses constitute the second process step consist of characteristics and factors that define the health condition of the patient. This is a clinical trial face to customer needs, family and community, which guides the nursing care towards efficiency, efficacy and effectiveness of assistance. To diagnose requires nurses critical thinking skills and technical-scientific knowledge, analysis, logical reasoning, clinical experience, patient knowledge, discernment, among others. In order to facilitate and standardize the language used in the formulation OF arise Classification Systems for Nursing Practice, highlighting the International Classification for Nursing Practice (CIPE) with international recognition. However, the use of rating systems and the implementation of the nursing process, in particular the construction of nursing diagnoses, still are configured as challenges to be overcome in the XXI century. There has been a shortage of professionals and the handling skills of taxonomies and clinical reasoning, indispensable to achieving the DE. This fact implies a reformulation as to the DE teaching methods, so that learning takes place effectively for students. In this context there are the active methodologies for the teaching of DE, with emphasis on realistic simulation that allows approach theory and practice, leading to greater satisfaction, learning, motivation, realism, confidence, improvement techniques, reflective thinking and transfers of competences between different realities. The study aims to: evaluate the effectiveness of realistic simulation in the teaching-learning process of nursing diagnoses of CIPE for undergraduate students in nursing. This is an experimental study of clinical trial type randomized controlled (RCTs). The research will be conducted at the Federal University of Nursing Department of Rio Grande do Norte (UFRN), Campus Christmas. Attend the undergraduate nursing study UFRN, which are on the 6th period of the course, after having attended the disciplines of semiotics and semiotics in nursing, methods and models in nursing care and comprehensive health care I. The control variables are: age, sex, clinical experience, occupation, student and IRA profile. Data will be collected from September 2016. Students will attend the course "Clinical Judgment and Reasoning Diagnosis: skills for learning of Nursing Diagnoses of CIPE", which will be developed in five stages: expository-dialogued lecture, simulation scenario 1, 2 and 3, evaluation of the course. At first all participants will complete a questionnaire to characterize as the sociodemographic and academic aspects. Then all students will participate in the exhibition-dialogic class, will be held after the pretest with all subjects, which will consist of a case for which they must draw the nursing diagnoses of CIPE. Subsequently there dividing the participants into two groups randomly with respect to control variables. The intervention group will be divided into smaller groups that will continue participating in the following stages of the course, simulation scenarios 1, 2 and 3, while the control group will be waived until the last day of the course. In the last meeting the two groups of the course will be gathered, then a post-test will be applied in which students will receive the same clinical case and should formulate nursing diagnoses grounded in CIPE, for comparison with the diagnoses validated by experts. The effectiveness of simulation strategy will be evaluated by the hit scores in both groups after the pre and post-tests. Subsequently, an instrument with questions about the degree of utilization of strategy and benefits for the development of nursing diagnoses will be applied, which will be answered on a scale type "Likert" five points. The socio-demographic data and academic of students and experts, the titles of the ICNP® diagnoses, validation scenarios and evaluation of teaching strategy will be organized in Microsoft Office Excel 2010® spreadsheet, expressed as means and standard deviations (simple descriptive statistics) . They will be organized in tables and analyzed based on absolute frequencies in measures of central tendency and dispersion, and association tests, the mean difference. For numeric variables, will be applied the Kolmogorov-Smirnov test to verify the normality of the numerical data and the Levene test to verify the homogeneity of variances. To assess the independence of the variables will be used Pearson's chi-square test and Fisher's exact test (when the expected frequencies are less than or equal to 5). To compare means, will apply the t test and Mann-Whitney test. To evaluate the intragroup mean differences will be used paired t test. In all inferential calculations will be adopted the significance level of 5% (p <0.05). How ethical to conduct research on human subjects, the study of the project will be forwarded to the Department of Nursing of the Federal University of Rio Grande do Norte to obtain the authorization of the institution and after their agreement will be sent to the Research Ethics Committee (CEP ) of the Federal University of Rio Grande do Norte (UFRN). Regarding the subjects who accept participate in the study will be used a free and informed consent, respecting Resolution 466/12, which guarantees the subjects anonymity and the right to withdraw from the research as well as for your interest.
