Myofibroblasts (MFs) and angiogenesis are important factors in the development and progression of cystic lesions, so that cells secrete growth factors and proteases, as well as angiogenesis stimulates matrix deposition and cell migration, affecting the growth of periapicopathies. This study aims to evaluate the immunohistochemical expression of CD34 and α-SMA in root cysts (CRs) and residual root cysts (CRRs) in order to contribute to a better understanding of the expansion and progression of periapical lesions. The present study constitutes a descriptive, quantitative and comparative analysis of the positive immunohistochemical expression of CD34 and α-SMA in the specimens on 30 CRs and 30 CRRs. Expression of α-SMA was evaluated in the fibrous capsule of the lesions, at a magnification of 100x below the epithelial lining 10 fields of higher immunolabeling were selected, thereafter at a 400X magnification, the positive cells were quantified and at the end were calculated the averages per field. However, the angiogenic index was performed by counting microvessels immunolabelled by the anti-CD34 antibody in 5 fields (200x). The results showed that there were statistically significant differences regarding α-SMA (p=0,035) immunostaining and the α-SMAXCD34 ratio (p = 0.050) in CRs and (p = 0.003) in CRRs, but the immunostaining for CD34 did not reveal statistical differences between the lesions (p=0,723). In conclusion, in view of the obtained results we consider that the significant correlation between the presence of myofibroblasts and the angiogenic index among the studied lesions are related to the expansion and growth of CRs and CRRs.
