Introduction: Actinic cheilitis (AC) is an inflammatory condition of the lips caused by chronic exposure to ultraviolet radiation and is considered an oral polenlially malignanl disorders by the World Health Organization (WHO). Objective: develop an Actinic Cheilitis Clinical Index (ACCI). Methodology: Based on a review of the literature, a questionnaire composed of 35 clinical characteristics of the QA was elaborated, which were selected and associated with a clinical severity score by a group of 52 dentists. After establishing inclusion and exclusion criteria, 16 characteristics were selected to compose the index. After ICQA application and incisional biopsy, 20 patients were selected to compose our sample. The biopsied specimens were classified according to the histopathological grading for oral epithelial dysplasia of WHO (EL-NAGGAR et al., 2017) and the Binary System (KUJAN et al., 2006). The association between the clinical characteristics and the degree of histopathological severity of the lesion was analyzed using Fisher's Exact Test (p <0.05). The accuracy of the index was tested based on the percentage of sensitivity, specificity, PPV, NPV, accuracy and ROC curve for cut off points 5, 7, 9, and 11. Results: There was a prevalence of males (80 (90%), white (90%), with history of smoking (73.7%) and occupational sun exposure (75%). There was no significant association between the clinical characteristics of QA and histopathological gradations. Conclusion: The ICQA proved to be a diagnostic instrument easy to apply, fast and low cost, which can be applied in patients with QA as a way of assisting the dentist to identify the cases that present indication of biopsy, as well as, in the choice of the area to be biopsied.
