Introduction: Systemic Arterial Hypertension (SAH) is one of the main Non-Communicable Chronic Diseases (NCDs), being associated with significant clinical, functional, and social repercussions. Beyond the biomedical approach, the understanding of functioning from the perspective of the International Classification of Functioning, Disability and Health (ICF) allows the analysis of the impacts of the disease on mobility, body functions, and social participation. Objective: To analyze the influence of Systemic Arterial Hypertension and personal factors on the levels of functional disability among residents of the municipality of Santa Cruz, Rio Grande do Norte, Brazil. Methods: This is a cross-sectional, population-based study with a quantitative approach, conducted with 504 adult and older participants living in Santa Cruz/RN, Brazil. Participants were divided into two groups according to the self-reported diagnosis of SAH. Data collection was carried out using the Model Disability Survey (MDS-Brazil), an instrument based on the biopsychosocial model of the ICF. Modules related to sociodemographic characteristics, functioning, and health conditions were used. Functional disability scores were estimated using Item Response Theory, applying the Rasch model for polytomous data, and subsequently transformed into a 0–100 scale. Statistical analyses included descriptive statistics, Student’s t-test, and multiple linear regression, adopting a significance level of 5%. Results: Individuals with SAH presented higher mean levels of functional disability when compared to non-hypertensive participants. Statistically significant differences were observed in the domains of mobility, body functions, and activities and participation. In the adjusted analysis, SAH remained associated with higher levels of disability in the domains of body functions and activities and participation. A higher frequency of limitations related to intense physical activities, prolonged standing, mobility, and performance of daily tasks was also observed among hypertensive participants. Conclusion: Systemic Arterial Hypertension was associated with higher levels of functional disability, especially in domains related to body functions and participation in daily activities. These findings reinforce the importance of incorporating functional assessment into Primary Health Care, contributing to the planning of more comprehensive care strategies, disability prevention, and promotion of individuals’ autonomy.
