Introduction: Hypertension (HTN) is characterized as a chronic health condition that affects individuals across different age groups, ethnicities, and socioeconomic backgrounds. This study fills a gap in the national literature by investigating the relationship between functionality and HTN from a population-based perspective, using an internationally validated instrument. It reinforces the relevance of generating local data to support public health actions aimed at promoting functionality.Objective: To describe and compare levels of disability between hypertensive and non-hypertensive individuals in the municipality of Santa Cruz, RN, Brazil.Method: This is a quantitative, cross-sectional, population-based study conducted in the municipality of Santa Cruz, RN. The target population consisted of adults aged 18 years or older, of both sexes, with or without a diagnosis of systemic arterial hypertension (HTN). Inclusion criteria were being a resident of the city, aged ≥18 years, and providing consent to participate in the study. Exclusion criteria included incomplete questionnaires or difficulty in responding. The sample consisted of 504 individuals, selected through intentional and random household sampling to ensure representativeness of the city’s different census sectors. Data collection took place between October and November 2022 through face-to-face household interviews. As the main investigation instrument, the Brazilian version of the Model Disability Survey (MDS-Brasil) was used, applied with the aid of tablets, allowing standardized collection of information on health conditions, sociodemographic aspects, and levels of functionality according to the principles of the International Classification of Functioning, Disability and Health (ICF).Results and Discussion: Regarding the sociodemographic characteristics of the group without HTN, a higher prevalence of females was observed, totaling 236 individuals (72.4%), while males accounted for 90 participants (27.6%). The mean age was 46 years. Concerning race/skin color, 119 individuals (36.5%) self-identified as White, followed by 156 (47.9%) as Brown, 43 (13.2%) as Black, and 7 (2.1%) as Yellow. A residual number of participants were unable to inform or identified with another category of race/skin color. In terms of education, 110 participants (33.7%) reported having incomplete elementary schooling, 69 (21.2%) reported having completed high school, and 37 (11.3%) stated they had no formal education. As for monthly income, most participants (68 individuals; 20.9%) reported earning less than the current minimum wage (R$1,200.00).The findings of this study indicate that hypertensive individuals exhibited higher levels of functional disability in the domains of mobility, body functions, and participation-related activities compared to non-hypertensive individuals. These results corroborate previous investigations demonstrating the direct influence of hypertension on functional limitations, affecting autonomy and quality of life. It is also observed that sociodemographic factors such as advanced age and low education levels further intensify these limitations, reinforcing the multifactorial nature of disability. Conclusion: These results reinforce the understanding that HTN negatively impacts multiple dimensions of human functionality, extending beyond clinical aspects and reaching psychological and social spheres.
