SUMMARY:

Abstract: Syphilis has been a simple, effective and relatively inexpensive treatment for more than three quarters of a century. However, both acquired and congenital syphilis have increased dramatically over the past two decades. In 2002, two out of every 1,000 live births in Brazil tested positive for syphilis. In 2019, the syphilis detection rate in pregnant women was 20.8/1,000 live births and the incidence rate of congenital syphilis was 8.2/1,000 live births. The numbers of cases of the infection are worrying, despite all efforts in recent years. The World Health Organization's goal is to reduce it to 0.5 per 1,000 live births, however, there is a lot of difficulty in achieving this goal, which is seen throughout history. Objective: This project aims to contribute to a theory of change using quality improvement tools to reduce vertical transmission of syphilis, increasing the timely diagnosis and treatment of pregnant women with syphilis. Methodology: This is the elaboration of a theory of change, built jointly by a multidisciplinary team from the Federal University of Rio Grande do Norte (UFRN) and the Institute for Healthcare Improvement (IHI), under the aegis of the Breakthrough Series Collaborative methodology of the IHI . A pilot study with a quasi-experimental design is under way, in which improvement teams from 10 municipalities in Brazil are testing the theory built. Once confirmed that this theory of change results in improvement, it will be available to be implemented for the care of people with syphilis. Results: The Guiding Diagram is organized into 5 primary drivers: 1) Pregnant women with access to prenatal care without barriers; 2) Uninterrupted flow of care for pregnant women with syphilis; 3) Highly trained teams in culturally appropriate care for pregnant women with syphilis; 4) Pregnant women, family members and sexual partners involved in the prevention and treatment of syphilis; 5) Leaders engaged in reducing syphilis during pregnancy; to which the 8 secondary drivers relate: 1) Co-create efficient mechanisms for scheduling and prenatal care that are culturally sensitive and adapt to the needs of different populations; 2) Make the VDRL and Rapid Test for syphilis available daily at the units; 3) Promote effective management of syphilis and reinfection cases; 4) Ensuring the availability and application of medicines; 5) Train health professionals in the application of prenatal and care protocols for pregnant women with syphilis; 6) Train health professionals in the application of culturally appropriate approaches to pregnant women and partnership with syphilis; 7) Develop culturally appropriate actions on syphilis for pregnant women, families and sexual partners. (Including the identification of intimate partner violence); 8) Create mechanisms for involving the leadership in monitoring the Collaborative's actions and results.

The Theory of Change Measurement Strategy was built in line with the objective of the project and the main processes involved, having determined outcome, process and balance indicators. This collaborative also fosters the formation of high-impact leaders who focus their efforts on achieving better health and better care for the population. Conclusions: The theory of change represents an opportunity to promote changes in the work processes of primary care teams for the care of pregnant women in prenatal care, with a view to reducing vertical transmission of syphilis and also providing learning about the fundamentals of the Science of Improvement and the IHI Improvement Model, which can be used in the future in other initiatives, inside and outside the selected units.
