Introduction: Anemia is a clinical condition characterized by a reduction in the amount of hemoglobin in the blood. It is considered a serious global public health problem, especially for pregnant women and children. Objective: To analyze the epidemiological profile, hospital costs, and mortality due to anemia in children and adolescents aged 0 to 19 years in Brazil, from 2013 to 2024. Methodology: This was a descriptive, ecological, retrospective study with a quantitative approach. It was conducted using secondary data from the Mortality Information System (SIM) and the Hospital Information System (SIH), available via DATASUS (Department of Informatics of the Brazilian Public Health System). Statistical methods were presented in tables, using absolute and relative frequencies for variable description. Data normality was assessed using the Kolmogorov-Smirnov test, and time series trend analysis was performed using the non-parametric Mann-Kendall test. Population values were used to create maps and calculate Moran's ($M$) and Geary's ($C$) indices to obtain spatial autocorrelation. The Local Indicators of Spatial Association (LISA) were used to evaluate local autocorrelation within the Brazilian federative units (UFs) and regions. Cost and hospitalization data were analyzed using the R language, where the total service cost value for each UF was divided by its respective Gross Domestic Product (GDP) and multiplied by $1,000,000.00$ (one million), generating the cost indicator per R$1,000,000.00$ of GDP for the period of 2014 to 2024. Data extraction took place between July 7th and August 13th, 2025. Results: A total of 3,815 deaths in children and adolescents due to the disease were reported, with the highest concentrations found in the Northeast Region (1,506 deaths, representing 39.5%). The States with the greatest significant increasing mortality trend, after applying the Mann-Kendall test, were Acre ($S=37$) and Amazonas ($S=29$). Regarding costs/investments from the Hospital Information System (SIH), the regions of the country with the highest total expenditures were the Southeast, with R$2.38 million, followed by the Northeast, with R$2.07 million. Conclusion: Mortality due to anemia in children and adolescents is concentrated and shows an increasing trend in the North and Northeast Regions, as evidenced by the temporal and spatial trend analyses, and is associated with high hospital costs. The findings indicate the need to prioritize and regionalize public health interventions for anemia control in the country.
