infarction based on a Middle-Range Theory.

SUMMARY:

Patients with acute myocardial infarction (AMI) are at imminent risk of losing their life or the function of an organ/system of the human body, developing states of shock and multisystem impairment. The identification of risk factors and conditions associated with the risk of shock allows directing nursing care to patients with AMI at risk of shock. It appears that at NANDA International this diagnosis was validated based on studies with patients in septic shock, hence the need to develop studies on patients with cardiological conditions in order to cover the concepts related to the diagnosis in this population. In this sense, the general objective of this study was to validate the content of the Nursing diagnosis Risk of shock in people with acute myocardial infarction based on a Medium Range Theory. This is a methodological study, which was carried out in two stages, which were: theoretical-causal validation and content validation. The research was approved by the UFRN Ethics Committee, through opinion no. 5,544,882 by CEP/UFRN and registered on Plataforma Brasil with the Certificate of Ethical Appreciation (CAAE) no. 58803922.1.0000.5537. Data analysis occurred using descriptive and inferential statistics. The first stage consists of building a mid-range theory operationalized through an integrative literature review. Data were extracted from the following data sources: Scopus, Web of Science, National Library of Medicine (MEDLINE/PubMed) and Science Direct. The final sample corresponded to 25 studies. Four attributes, 7 risk factors and 13 conditions associated with the risk of shock in people with acute myocardial infarction were listed. Then, the Medium-Range Theory of Stress Risk of shock in people with AMI (TMA-RCIAM) was constructed, the theoretical reference of Roy's adaptation model. 20 concepts, 7 propositions and 23 causal relationships between the risk of shock and patients with acute myocardial infarction were identified. A graphical representation was constructed to explain the relationships between the concepts. The TMA-RCIAM evaluation was carried out, which took place in two rounds. 49 judges participated. In the first round, the TMA-EOPS was considered adequate regarding all items related to significance, internal consistency, parsimony, testability and empirical adequacy and pragmatic adequacy.However, the items were adjusted in order to accept comments and suggestions, and were reevaluated in the second round of evaluation. Finally, approval was obtained on all items. The second stage involved content validation by judges regarding the diagnostic components. 62 judges participated in this stage, who validated the definition of the diagnosis and the 20 clinical indicators as to their relevance. The conceptual and operational definitions of each indicator were assessed for clarity and precision. All items showed agreement above 0.8, but adjustments were made based on the evaluators' comments and suggestions. After the adjustments, a second round of evaluation was carried out with the judges, in which agreement was obtained. It is concluded that the study contributed to the advancement of knowledge in the discipline of Nursing through the development of nursing diagnosis concepts already existing at Nanda Internacional, but with a focus on patients with myocardial infarction.
