SUMMARY:

Patients in the postoperative period of cardiac surgery have a high incidence of the nursing diagnosis Impaired physical mobility. In this context, it is necessary to analyze the evidence of validity of this diagnosis in this population. To this end, the objective was to organize the evidence of theoretical-causal validity, through a middle-range theory, of this diagnosis in this population and to establish the evidence of content validity of the elements by experts. This is a methodological study carried out in two stages: Theoretical-causal validation, through the construction of a middle-range theory; and content analysis. In the first stage, the middle-range theory was developed from an integrative review in four databases. The essential attributes, clinical antecedents, clinical consequences, their conceptual and operational definitions, propositions and causal relationships, and evidence for practice were identified. In the second stage, the diagnosis was analyzed by 09 experts, through the nominal group. The elements of the diagnosis were evaluated according to permanence in the study and adequacy in its construction. The research was approved by the Ethics Committee of the responsible institution, through opinion 4.039.222 and CAAE: 68761823.0.00005537. The results showed that the following were identified in the 19 selected articles: 03 essential attributes: Difficulty moving, Difficulty changing position and contracting muscles freely, Dependent physical movement; 04 clinical antecedents: Reduced tolerance to physical effort, Angina pain, Muscle weakness and Prolonged time of bed restriction; and, 07 clinical consequences: Change in gait, Difficulty turning, Reduction in range of motion, Limited muscular capacity to perform fine motor activities, Limited muscular capacity to perform gross motor activities, Dyspnea on exertion and Acute pain. A pictogram, 5 propositions and 10 causal relationships were developed. In the second stage, the analysis of the diagnosis by experts allowed the improvement of the title, concept of the diagnosis, 4 antecedents and 3 clinical consequences. This resulted in a diagnostic structure with 7 defining characteristics, 5 related factors, 1 population at risk and 1 associated condition. It was concluded that the middle-range theory and content analysis highlighted important elements for the diagnosis of Impaired Physical Mobility in patients undergoing cardiac surgery, which are not yet present in NANDA International. Therefore, it is expected to contribute to the advancement of the work process of nurses, through a more precise and reliable identification of the diagnosis of Impaired Mobility in this population. In addition, it seeks to promote the strengthening of practices aimed at the development of measures that promote early mobilization and reduction of injuries aimed at the rehabilitation of patients in the postoperative period of cardiac surgery.
