SUMMARY:

This study is a review that sought to explore the current landscape of the medicinal use of Cannabis sativa L., focusing on its use as an active pharmaceutical ingredient, quality control methodologies, and therapeutic applications. Through a review of the literature, publications between 2016 and 2026 in the ACS, PubMed, and Web of Science databases were considered, and the study evaluated analytical techniques and formulation trends. The results indicate that High-Performance Liquid Chromatography (HPLC), particularly with UV/Vis detectors such as DAD, is the gold standard for quantifying cannabinoids, while Gas Chromatography coupled with Mass Spectrometry (GC-MS) is the preferred tool for terpene analysis. For comprehensive, non-targeted analyses, techniques such as NMR and High-Resolution Mass Spectrometry (HRMS) are increasingly used, along with rapid, low-cost alternatives such as NIR spectroscopy and smartphone-detection TLC. In product development, extraction methods rely heavily on ethanol or olive oil, especially for galenic and artisanal preparations. A critical step in this processing is thermal decarboxylation, which converts biologically inactive cannabinoid acids (such as THCA and CBDA) into their active neutral forms (THC and CBD). Although conventional oil-based extracts dominate the market, there is a significant shift toward innovative drug delivery systems designed to overcome problems such as low bioavailability and dose variability. Advanced formulations, such as nanoemulsions, nanostructured lipid carriers (NLCs), extracellular vesicles, and even 3D printed buccal films, have been developed to increase treatment adherence and efficacy. Therapeutically, these products show promise for chronic pain management, epilepsy, neurodegenerative diseases, and dermatological conditions. The study also discusses the therapeutic relevance of the “entourage effect” and suggests that the presence of terpenes may be questionable depending on the proposed use of the formulation. Thus, the current landscape of analytical methods shows a consolidation of chromatographic separation methods and comparison with spectroscopic methods. For the pharmaceutical technology area, incorporations in oil bases are predominant, while specialized formulations according to the perspective of use represent a field of study that is still largely unexplored in cannabis medicine.
