SUMMARY:

Abstract: Congenital cytomegalovirus (cCMV) is considered the main risk indicator for sensorineural
hearing loss among congenital infections. This dissertation comprises two studies. Article 1 aims to map and
characterize the procedures used in the audiological diagnosis of infants up to 24 months of age, describing
the sequence of examinations in specialized services. Article 2 seeks to investigate the neurophysiological
integrity of the peripheral and central auditory pathways in infants with cCMV. Method: Article 1
corresponds to an ongoing scoping review conducted in the databases PubMed, Web of Science, LILACS,
SciELO, Embase, Scopus, Google Scholar, and ProQuest Dissertations and Theses (PQDT), guided by the
question: “Is there consensus regarding the procedures performed in the audiological diagnosis of infants
aged 0 to 24 months?”. Article 2 consists of a cross-sectional, observational, and prospective study, approved
by the Research Ethics Committee of the Onofre Lopes University Hospital (No. 7.190.153). To date, 256
infants have been recruited from a public maternity hospital in Natal. Participants were divided into three
groups: G1 (cCMV, positive urine PCR up to 21 days), G2 (probable perinatal CMV, positive PCR after 21
days), and G3 (control, negative PCR). All infants underwent transient evoked otoacoustic emissions
(TEOAE), tympanometry, click and frequency-specific Auditory Brainstem Response (ABR), as well as
behavioral assessment (Ling sounds, attention, sound localization, and cochleo-palpebral reflex). Partial
Results: The scoping review is in the final phase of protocol development and registration. In Article 2, 19
infants have been included to date (G1 = 5; G2 = 5; G3 = 9), aged 1 to 6 months (34.8 ±0.855 weeks). In G1,
three infants (60%) presented absent TEOAEs in at least one frequency band; one (20%) did not undergo the
test, and one (20%) had bilateral present responses. All showed bilateral positive-peak tympanograms. In
click-ABR at 70 dB, waves I, III, and V were observed with normal latencies and interpeak intervals. In
frequency-specific ABR (500, 2000, and 4000 Hz), four infants (80%) presented normal thresholds, while
one (20%) showed conductive alteration at 500 Hz (air conduction = 45 dB, bone conduction = 20 dB nHL).
In G2, three infants (60%) presented absent TEOAEs in at least one frequency band, while two (40%) had
bilateral present responses. In click-ABR, all exhibited waves I, III, and V with normal latencies, and in
frequency-specific ABR all showed normal thresholds at the three tested frequencies. Data from the control
group (G3) are still under analysis. Conclusion: In these partial results, no sensorineural hearing loss was diagnosed in infants with CMV. However, the frequent occurrence of absent TEOAEs in at least one
frequency band in both groups reinforces the need for continuous audiological monitoring, considering the
risk of late-onset or progressive hearing impairments associated with CMV.
