SUMMARY:

Dysphagia is a condition often associated with Parkinson's disease and can cause not only a decline in quality of life, but also malnutrition, dehydration and fear of eating. It can also increase the risk of aspiration pneumonia, the main cause of death in these patients. Although there is drug treatment for motor symptoms, increasing the doses does not guarantee an improvement in swallowing disorders. Among the interventions for oropharyngeal dysphagia, those based on the chin-down postural maneuver stand out, which have demonstrated positive effects on the efficiency and safety of swallowing in patients with Parkinson's disease. However, other interventions are being developed, such as the Chin Tuck Against Resistance technique, a promising therapeutic method to rehabilitate swallowing function, although there are still no published data on its effectiveness in the Parkinson's population. Therefore, this research proposes to investigate the effectiveness of an intervention program based on Chin Tuck Against Resistance exercises, compared to the chin-down postural maneuver, in dysphagic patients with Parkinson's disease. Ethical principles were followed and the project was approved by the Research Ethics Committee. The first study is a controlled, randomized, blinded, parallel longitudinal clinical trial, currently in the data collection phase. Twenty-six participants will be randomly assigned to two groups for a four-week intervention period. The primary outcomes evaluated will be: (1) functional swallowing assessment; (2) therapeutic efficacy; (3) oral intake scale; (4) and maximum isometric tongue pressure. Secondary outcomes will include: (1) peak cough flow; (2) peak expiratory flow; (3) and vocal acoustic signal. The second study, which is longitudinal in nature and is currently in the data collection phase, will compare the progression of isometric tongue pressure across sessions in both intervention programs. To date, 18 individuals treated at the Onofre Lopes University Hospital, Natal (RN), Brazil, have been collected. Once data collection is complete, the research is expected to provide new evidence for the treatment of dysphagia in Parkinson's disease, as well as open new perspectives for intervention programs in oropharyngeal dysphagia.
