DEVELOPMENT AND VALIDATION OF AN INSTRUMENT FOR THE TRANSITION OF PATIENT CARE IN AN ADULT INTENSIVE CARE UNIT

Hospital healthcare services have incorporated organizational and technological innovations
with the goal of improving the quality of care and patient safety from the moment of admission to hospital discharge. However, the transition of care, especially during intra-hospital transfers, constitutes a critical moment in the care process, as it is subject to communication failures, loss of relevant information, and discontinuity of care. Thus, this research aims to develop a checklist for the transition of care for patients from the adult intensive care unit to the ward in the context of a general hospital. This is a methodological study, with a quantitative approach, which adopted as a theoretical framework the model of nurse Afaf Ibrahim Meleis, as it conceptually grounds the transition of care. As a methodological framework, Design Thinking will be used, a user-centered approach that guides the development of solutions from the understanding of the context, according to the stages of immersion, problem definition, ideation, prototyping, and testing. The study will be developed in the
Adult general Intensive Care Unit located at the Deoclécio Marques de Lucena Regional Hospital, a public institution of reference in the health care network, located in the municipality of Parnamirim, state of Rio Grande do Norte. The data collected in the empathy stage will be analyzed by simple descriptive statistics, as well as the findings of the integrative literature review. The validation data will be used to calculate the Content Validation Coefficient with a minimum of 70% agreement. The study will be submitted to the Research Ethics Committee of the Federal University of Rio Grande do Norte, following the guidelines of Resolution No. 466/2012 of the National Health Council.
