CONSTRUCTION AND VALIDATION OF AN EDUCATIONAL GUIDE FOR OUTPATIENT MEDICAL CARE FOR CHILDREN AND ADOLESCENTS WITH PRECORDIAL CHEST PAIN
Precordial chest pain. Pediatrics. Primary Health Care. Educational technology. Instrument validation.
In pediatrics, chest pain is a frequent complaint and presents a variety of manifestations, which can be described as tightness, stabbing, discomfort, and even "burning." Primary Health Care, as the entry point to the Brazilian Unified Health System (SUS), plays a strategic role in the initial assessment of chest pain in children and adolescents. However, evidence points to difficulties in conducting the specific anamnesis, insecurity in identifying signs of severity, and variations in referral decisions. The present study aims to construct and validate an educational technology in the form of a booklet to guide physicians in the outpatient management of precordial chest pain in children and adolescents. This is a methodological research, with a quantitative approach, developed in three poles according to Pasquali (2010): theoretical, empirical, and analytical. In the Theoretical Pole, a scoping review will be conducted according to the Joanna Briggs Institute and PRISMA-ScR, a situational diagnosis in a pediatric cardiology outpatient clinic, and the elaboration of a construct representing clinical competence in the assessment of chest pain. In the Empirical Phase, the booklet will be validated for content, relevance, clarity, and appearance by 12 expert judges using the Delphi technique and the Likert scale, and submitted to semantic evaluation by four pediatric residents through brainstorming. In the Analytical Phase, the Content Validity Index and the average Kappa Coefficient will be calculated to assess the agreement between experts and the consistency of the instrument, ensuring reliability and psychometric validity. This study was approved by the Ethics and Research Committee of the Federal University of Rio Grande do Norte, via Plataforma Brasil, with Opinion No.: 7.961.386, CAEE: 75875823.2.0000.5292. It is expected that the booklet will contribute to standardizing the clinical assessment of precordial chest pain in pediatrics, strengthening the effectiveness of Primary Health Care, reducing unnecessary referrals, and promoting evidence-based clinical decisions, addressing a gap identified in the Brazilian literature.
