Evaluation and Improvement of the Assistance to the Syphilis in Pregnancy in the Primary Health Care.The elimination of congenital syphilis is a priority for the Latin American and Caribbean region and is contained in the document Elimination Initiative of the Pan American Health Organization (PAHO) and the United Nations Children's Fund (UNICEF). Syphilis in pregnancy leads to more than 300,000 fetal and neonatal deaths per year in the world and puts an additional 215,000 children at increased risk of premature death. In Brazil, from 2011 to 2016, there is a considerable increase in the number of cases of syphilis in pregnant women throughout the country (129,727 cases), indicating an improvement in the epidemiological surveillance system and an increase in access to diagnosis. Among the units of the federation, Rio de Janeiro has the highest incidence rate of congenital syphilis, with 12.4 cases per thousand live births, while the national rate is 6.5 cases per thousand live births. Between January 2008 and October 2013, 6,754 cases of syphilis in pregnancy were reported and 6,064 cases of congenital syphilis were reported in residents of the city of Rio de Janeiro, with the highest number of cases recorded in Planning Area (AP) 3.3, north zone of the city. Considering the Primary Health Care in the city of Rio de Janeiro, it is observed that management contracts with Social Organizations (OS), starting in 2016, include in the matrix of OS performance evaluation the indicator "Proportion of notifications of syphilis in pregnancy with appropriate treatment". Based on the analysis of the results achieved for this indicator in AP 3.3, very distant from the proposed quality goals and standards, it was decided to develop a cycle of quality improvement for the problem identified with the objectives of assessing the quality of care provided to pregnant women diagnosed with syphilis in relation to adequate treatment of the disease, identify intervention priorities to improve the quality of syphilis treatment during pregnancy, and test the effect of a proposed intervention to improve the quality of care for pregnant women diagnosed with syphilis. For that, a quasi-experimental study of the type before and after in 6 stages was elaborated: 1. Identification and prioritization of the quality problem; 2. Analysis of the problem; 3. Development of quality assessment criteria; 4. Assessment of the level of conformity of the criteria; 5. Improvement intervention addressed to the criteria identified as priorities; 6. Reassessment of the quality level to verify the effectiveness of the intervention applied. The study was carried out at the Primary Care Units of the AP 3.3 of the city of Rio de Janeiro, based on the analysis of a sample of the medical records of the pregnant women attended. Preliminary results, obtained from the evaluation of the quality level, indicate the degree of compliance of quality criteria for syphilis care during pregnancy. Of the ten criteria analyzed in this study, the ones with the lowest compliance rate, therefore considered as priorities for intervention, were the partner's testing and treatment, adequate registration in the electronic medical record, adequate VDRL replication, and National Disease Surveillance Data System (SINAN) and International Classification of Diseases Codes (CID) records in time timely.
