EVALUATION OF WOMEN’S HEALTH CARE IN THE PREGNANT AND PUERPERAL CYCLE – AN ANALYSIS OF THE NATIONAL PRIMARY CARE ACCESS AND QUALITY IMPROVEMENT PROGRAM (NPCAQIP)

RESUMO:

In the process of construction of the Single Health System (SUS) the importance of the National Primary Care (NPC) is bigger and more evident. The teams of the primary care must be responsible for the population in their coverage area, maintaining a coordinated care. Within the high priority actions of the Family Health Strategy is prenatal care which aims to assure the good development of the pregnancy, allowing the birth of a healthy child. The present study aimed to evaluate women’s health care in the pregnant and puerperal cycle in the Northeast of Brazil through the data of the NPCAQIP. It is an evaluation study, with quantitative approach. The data used was obtained through the external evaluation bank of the NPCAQIP from both cycles provided by the Ministry of Health from the dimension of the women’s health care in the pregnancy and puerperium on the three modules of the data collection instrument, which are: Module I – Observation in the health unit, module II – Interview with a professional of the NPC team and verification of documents in the health unit and Module III – Interview with a client in the health unit. In order to analyze the data with a quantitative approach, the information was inserted in a data bank of the Statistical Package for Social Sciences (SPSS®) in the 17.0 version. After that, it was carried out a linkage of the two created data banks, one for the 1st cycle and the other one for the 2nd cycle, using the CNES variable and it was analyzed the same health units on both cycles, each cycle containing 9296 health units. The number of participants who took part in the study was 5146 health care professionals in each evaluation cycle, and regarding the clients, 1732 women on the first cycle and 2004 women on the second one took part answering questions concerning prenatal care. For the variable concerning the puerperium, 1743 women took part on the first cycle and 1952 women took part on the second one. The results were presented and discussed in different dimensions related to structure, process of work in the prenatal and process of work in the puerperium. It is highlighted the important consistence of the information about the pregnant card, prenatal tests, vaccination, postpartum follow-ups and Community Health Agent visit on the first week postpartum, considering they are variables which interlink with each other and are related by different individuals in the process. When it is compared to the second cycle of the NPCAQIP considering the variables of the woman in a pregnant and puerperal cycle, in relation to infrastructure, work process of the teams and perception and satisfaction of the clients, some advances can be noticed. Even though there are some variables which diminished or remained the same from one cycle to another, it is clear that the proposal of change induction in the practices of service, which was proposed by the NPCAQIP, is slowly consolidating. The evaluation cycles aim to encourage and follow the development of the different individuals involved in the search for better quality of health. One suggests that at every cycle the data collection instrument is enhanced and that basic criteria for certification is created.
