Infant Death Rate (IDR) consists in one of the most used indicators to analyze public health situation. Identifying risk factors related to that may help on planning actions for restructuring and improving maternal and infant care, aiming death cases reduction. This is a study of descriptive character, within a quantitative approach, that has the goal to analyze the quality of investigations on infants and fetus death cases in Caicó/RN context. As research participants, all cases of infant or fetus death (N=55), notified and inquired on the Mortality Information System (MIS) from 2010 to 2015 in Caicó, were included. A secondary data about the infant and fetus cases, from death surveillance sector of the city and the MIS, was used on this study. In order to achieve a better analyzes of the investigations, the research analyzed the following dimension: Completeness, Consistence, Eventuality classification and investigation time length, each of those with its variable of study. This data was analyzed through absolute and relative frequency using the statistical software Statistical Package for the Social Science (SPSS) version 22.0. The study result showed a low percentage of investigation (45.3%) on the study time period, with most of the investigation happening way beyond the opportune deadline, stipulated by the Health Ministry.  Those were not done in an effective way, once was verified a low completeness and presence of innumerous inconsistencies. From the total of infant and fetus death cases investigated, the majority of the them (64.8%) were avoidable by health service action, and only 13.7% were in concordance when comparing the results based on the Brazilian List of Preventability and the investigation performed by the county. In overall, this study pointed a fragility on the death investigation in Caicó, mainly on the context of investigation closure. Therefore, it is important to reinforce the role of the investigation committee and further, highlight investigation records as an important tools for decision making management and infant mortality tracking, requiring additional investments, as capacitation for health care professionals, in order to overcome the identified problems.
