Continuous Improvement Cycle for Transfusion Safety in Elderly Person

Introduction: The aging population results in an increased demand for health services, especially in Hemotherapy for the elderly. This age group presents physiological changes, such as reduced cardiac reserve, which increases the risk of transfusion reactions, particularly Transfusion-Associated Circulatory Overload (TACO). The lack of specific care protocols for geriatrics contributes to the underreporting of transfusion-related adverse and unsafe practices. Objective: to implement an improvement cycle for transfusion safety in the elderly population at a public teaching hospital. Specific objectives include interventions for the prevention of circulatory overload in the elderly, analysis of quality indicators, and team adherence. Methodology: This is a quasi-experimental (before-and-after, without a control group) study with a quantitative approach, based on the SQUIRE 2.0 guidelines. The setting is the Hemotherapy Unit of a university hospital in Rio Grande do Norte. The intervention will consist of three main actions: the adaptation of the institutional protocol (incorporating risk screening and clinical decision tools); training workshops for the healthcare staff based on Permanent Education; and the implementation of the protocol as a pilot project in the wards, supported by the donation of vital signs monitoring equipment. The staff sample will include 17 members of the hemotherapy team, and the patient sample will be a convenience sample consisting of 50 medical records of elderly patients (⩾ 60 years old) who received packed red blood cells, divided equally into two groups: 25 in the pre-intervention period (baseline) and 25 in the post-intervention period. Data will be extracted from the AGHU and Soul MV electronic systems and analyzed using descriptive statistics and the Pareto Diagram quality management tool. The project was approved by the Research Ethics Committee (Opinion No. 8.476.486). Expected results: The application of the new tools is expected to increase compliance in medical records, reduce the incidence of TACO in the elderly population by 30% within 12 months, mitigate the underreporting of immediate reactions, and promote the consolidation of a patient safety culture in geriatric hemotherapy.
