SUMMARY:

Introduction: Patient safety is recognized as a global strategic priority, with emphasis
on strengthening reporting and learning systems based on adverse events. In Brazil,
although national guidelines have established the implementation of Patient Safety
Centers and incident reporting systems, institutional responses to reported events
remain fragile, compromising the consolidation of a safety culture and organizational
learning. Objectives: To implement and evaluate a quality improvement cycle aimed at
strengthening the institutional capacity for response and learning from adverse events in
a large philanthropic hospital in Ceará, Brazil. Methods: This is a mixed-methods study,
structured according to the SQUIRE 2.0 guidelines, developed in two sequential and
complementary phases. The qualitative phase, with a descriptive-exploratory design,
will include semi-structured interviews with key stakeholders, document analysis, and
application of the CFIR and MUSIQ frameworks to understand the organizational
context, identify barriers and facilitators, and assess readiness for implementation. The
quantitative phase will adopt a quasi-experimental before-and-after design, using
anonymized secondary data from the institutional reporting system and a perception
survey with healthcare professionals. Process and learning indicators will be evaluated,
including timeliness of investigation, feedback to reporters, completion of case
management, and effectiveness of action plans. Expected results: The intervention is
expected to strengthen the institutional reporting and learning system by improving the
standardization of post-reporting processes, enhancing the quality of action plans,
increasing leadership accountability, and consolidating indicators capable of monitoring
the effectiveness of institutional responses to adverse events. Conclusions: The
proposed methodological approach will enable a comprehensive understanding of the
organizational context and allow systematic evaluation of the intervention’s effects,
contributing to the strengthening of patient safety culture and the production of applied
evidence in large philanthropic hospitals.
