Introduction: Climate change and its impacts on health have intensified the need to incorporate sustainable practices into healthcare services, requiring the integration of care quality, patient safety, and environmental responsibility. In this context, digestive endoscopy stands out as an area with high environmental impact due to the large volume of procedures, intensive use of disposable materials, and significant resource consumption. This highlights the relevance of the green endoscopy concept, which promotes sustainable practices without compromising the quality of care. Objective: To implement a protocol of sustainable practices in digestive endoscopy through the measurement of environmental, clinical, and operational indicators. Methods: This is a quasi-experimental, before-and-after study without a control group, with a quantitative approach, developed based on a quality improvement cycle in accordance with SQUIRE 2.0 guidelines. The study will be conducted in an outpatient digestive endoscopy service, including analysis of procedures, medical record audits, structural and care process evaluations, and training of healthcare professionals. Quality criteria will be established based on the Avedis Donabedian model, organized according to ESG pillars and operationalized through measurable indicators, including waste generation, estimated carbon footprint, clinical appropriateness of exams, and compliance with care practices. Results: The study is expected to identify gaps related to sustainable practices within the service, as well as to measure the environmental impact of endoscopic procedures. The implementation of interventions is anticipated to improve waste segregation, optimize resource utilization, and enhance the organization of care processes. Conclusions: The adoption of sustainable practices in digestive endoscopy, through a structured protocol based on measurable indicators, may contribute to reducing the environmental impact of procedures, improving quality of care, and strengthening patient safety. Green endoscopy is thus reinforced as a feasible and effective strategy within healthcare services.
