Introduction: Low adherence to patient safety practices in hospital care persists even in the face of a robust regulatory framework, such as Collegiate Board Resolutions 63/2011 and 36/2013 of the National Health Surveillance Agency, and Ministry of Health Ordinance 529/2013, which establishes the National Patient Safety Program in Brazil. Several studies highlight failures in the application of essential patient safety measures, associated with the absence of an institutional safety culture, low strategic prioritization of the topic by management, scarcity of continuing education, work overload, and reduced leadership engagement. In this scenario, research is needed to deepen the understanding of these barriers and the factors that favor adherence to safe practices in the real context of services, especially in critical environments such as intensive care units. Objective: To identify the barriers to adherence to patient safety practices in hospitals with intensive care beds in the state of Mato Grosso, in light of implementation science. Method: This was a descriptive, qualitative study conducted in hospitals with intensive care units in the state of Mato Grosso, Brazil. Participants were professionals from the Patient Safety Centers of these institutions. Data collection took place in 2024 using an electronic form with four guiding questions about challenges in different dimensions of care. The responses were examined, organized, and categorized according to Implementation Science, using the Consolidated Framework for Implementation Research (CFIR) instrument. The project has been approved by the Ethics and Research Committee of the Federal University of Rio Grande do Norte (CEP/UFRN), as per substantiated opinion No. 7.190.121, CAAE: 83375624.9.0000.5292. Results: The findings reveal that the implementation of patient safety in hospitals with intensive care beds in Mato Grosso is limited by structural weaknesses, scarcity of resources, high professional turnover, low training, communication failures, a punitive culture, and low leadership engagement, compromising adherence to patient safety protocols within these services. Conclusion: Based on the observations, it is concluded that patient safety requires investment, active leadership, continuous training, and strengthening of the organizational culture, far beyond the mere existence of formal protocols.
