SUMMARY:

Introduction: Falls are one of the most frequent adverse events in the hospital setting, causing severe harm to patients and compromising the quality of care. To address this issue, nurses utilize the Morse Fall Scale (MFS), a validated instrument that stratifies risk and supports the implementation of preventive measures. However, knowledge gaps and difficulties in the practical execution of this tool limit the effectiveness of safety strategies and jeopardize the integrity of the care provided. Given this scenario, education on the subject is an essential strategy. In this context, serious games emerge as a technological tool for educational purposes—rather than just entertainment—allowing healthcare professionals to practice the application of the MFS and fall prevention measures within a safe virtual environment. Objectives: To develop the Game Design Document (GDD) for the serious game ‘Caindo na Prevenção’ (Falling for Prevention). Method: This is a methodological study based on the instructional design framework, which focused on developing an educational technology tailored to the learning needs of 33 preceptor nurses and nursing residents from the Onofre Lopes University Hospital (HUOL) who participated in the research. The study was structured into two interdependent stages. The first consisted of a situational diagnosis, which identified the main knowledge gaps and practical difficulties of the nursing team regarding fall management. The second stage used these data to draft the Game Design Document (GDD), a phase in which the study defined the mechanics, script, and pedagogical elements of the serious game ‘Caindo na Prevenção’. Results: The combined percentage of participants who had never attended training on fall prevention (27.3%) and those who had attended only one (33.3%) reveals that 60.6% of the sample has minimal or no participation in the topic. This data highlights a critical point and suggests a gap in the continuous training of professionals. The Game Design Document (GDD), the educational product of this research, incorporates gamification elements such as scoring systems, rankings, and immediate feedback, while utilizing pixel art aesthetics. This ensures a user-friendly reception and a reduction in technological resistance, enhancing engagement and fluidity across devices. The structuring of the document addressed the real needs of healthcare practice and regulatory documents, which will contribute to ensuring clinical applicability and relevance within the hospital setting. Conclusions: The study demonstrates that the Game Design Document (GDD) serves as an indispensable methodological foundation for the creation of the serious game, establishing it as an essential tool in digital game development. This document systematically consolidates technical, aesthetic, and functional requirements, ensuring pedagogical fidelity and minimizing interpretational errors by the development team. By aligning the project's vision with learning needs, the Game Design Document (GDD) ensures that the final product precisely addresses the real demands of clinical practice.
