SUMMARY:

In recent years, Brazil has undergone a dismantling of health policies, producing a series of consequences in the field of mental health. One dimension of this problem concerns the relationships among services, professionals, and users with the logics governing psychotropic drug prescription, particularly insofar as medication management is considered a non-reformed dimension of the Brazilian Psychiatric Reform. A health strategy that sheds light on this context and seeks to intervene in it is Autonomous Medication Management (Gestão Autônoma da Medicação – GAM). Considering that Brazil is a country of continental dimensions, marked by socioeconomic, cultural, and structural inequalities in the provision of health services, significant gaps remain both in Brazilian GAM research across different territories and in the broader process of the Brazilian Psychiatric Reform itself. Accordingly, this study aims to carry out an intervention using the GAM strategy at the CAPS II in Santa Cruz, Rio Grande do Norte, seeking to analyze modes of care directed at subjects labeled as “mad” in this city. More specifically, the study seeks to foster the production of autonomy and co-management among users, to problematize processes of medicalization in mental health, and to collectively construct transformations in the GAM strategy based on the particularities of the territory. To achieve these objectives, a research-intervention was conducted articulating the GAM strategy with psychoanalysis, a perspective that guided both the facilitation of the group and the critical examination of mental health care and medicalization processes present in the territory. Together with the group participants, mediating objects were developed in order to incorporate the cultural specificity of the territory into the GAM framework, in dialogue with the existing instrument. The construction of these mediators was grounded in an archaeological approach to the history of the territory of Santa Cruz, attentive to transferential relations, unconscious alliances, and prevailing modes of relating to individuals labeled as mad within the service and the city. The lived experience of the group was documented through the writing of collective memories, developed collaboratively with the group and during supervision meetings with the research team, the advisor, and CAPS professionals. Twenty meetings were held, involving eight users, three undergraduate students, and three CAPS professionals. The most recurrent themes included users’ experiences of exclusion within the city, the violence they faced, reports of violated rights, and their relationships with psychotropic medications. The results of the experience highlight the constitution of the GAM group as a space for the construction of liberating bonds, as well as the creation of mediating objects as an alternative for adapting GAM for users with limited literacy or low familiarity with reading practices.
