SUMMARY:

Introduction: The culture of care relates to mutual appreciation among workers, peer support, and an institutional commitment to professional well-being, all of which impact the quality of care, job satisfaction, and the sustainability of healthcare organizations. To date, there is no validated instrument to assess the culture of care in Brazil. Given this, the aim of this study was to cross-culturally adapt the Culture of care barometer instrument for Brazilian culture and language. Methods: This research comprised two methodological designs: a scoping review (studies 1 and 2) and a methodological study involving cross-cultural adaptation, psychometric validation, and application of the instrument (studies 3, 4, and 5). Searches for the scoping review were conducted in the PubMed, LILACS, Cochrane Library, and Embase databases, including studies addressing the culture of care in healthcare services. The methodological study was conducted in two stages: cross-cultural adaptation of the instrument and application of the instrument for psychometric validation. The cross-cultural adaptation process included translation, synthesis, back-translation, expert evaluation, and pre-testing, based on the model by Beaton et al. (2000). The phase involving the application of the instrument and psychometric validation was conducted with 306 participants. Data collection took place between May and June 2026; the data were used to evaluate evidence regarding the internal structure validity of the Brazilian version of the Culture of care barometer (CoCB-BR). This study was approved by the Research Ethics Committee of HUOL/UFRN under CAAE No. 74738523.4.0000.529. Results: In the scoping review, 3,289 references were identified, and 7 met the inclusion criteria. A manual search identified 22 studies, 15 of which met the inclusion criteria, resulting in a total of 22 studies analyzed. The results show that the literature focuses on three main areas: the construction and validation of instruments to measure the culture of care; theoretical reflection and methodological synthesis regarding the culture of care; and the assessment of the culture of care. The results of the methodological study indicate a high level of agreement among experts regarding most items. Pre-testing revealed high levels of item comprehension and relevance, indicating adequate clarity, semantic equivalence, and applicability of the instrument within the Brazilian context. Furthermore, the instrument demonstrated adequate factorability (KMO=0.899; Bartlett p<0.001), high reliability (ω=0.963), andsatisfactory adjustment rates. The internal structure revealed two factors that grouped together the items from the four factors of the original version. Final considerations: The Brazilian version of the CoCB-BR demonstrated satisfactory evidence of validity based on internal structure and high reliability, proving suitable for measuring the culture of care in Brazilian healthcare institutions. It is expected to contribute to strengthening research and practices aimed at assessing the culture of care in health services, thereby supporting workforce management strategies, the promotion of healthier organizational environments, and improvements in the quality of care.
