SUMMARY:

The aim of this study was to investigate the influence of psychosocial factors (sense of coherence, self-esteem, and perceived stress) on self-reported periodontal disease among Brazilian individuals using Structural Equation Modeling (SEM). This was an observational, quantitative, cross-sectional study conducted with 408 individuals aged 18 years or older who sought dental care at the School of Dentistry Clinic of the Federal University of Rio Grande do Norte (UFRN). Sociodemographic, behavioral, and systemic health data were collected using standardized questionnaires. Psychosocial factors and self-reported periodontal disease were assessed using instruments validated for the Brazilian population: the 13-item Sense of Coherence Scale (SOC-13), the Rosenberg Self-Esteem Scale, the Perceived Stress Scale, and the Brazilian version of the Oral Health Questions Set B (OHQB-Br). Data analysis included descriptive statistics and multivariate analysis using Structural Equation Modeling (SEM) to evaluate the direct and indirect effects among the investigated variables. Results were expressed as standardized beta coefficients and p-values, adopting a significance level of 5%. Sense of coherence (β = 0.867; p < 0.001) exhibited the highest factor loading on the latent construct "psychosocial factors," followed by perceived stress (β = −0.622; p < 0.001) and self-esteem (β = 0.423; p < 0.001). Psychosocial factors showed a significant direct negative effect on self-reported periodontal disease (β = −0.188; p = 0.028), indicating that poorer psychosocial conditions were associated with a higher likelihood of periodontitis. The findings also demonstrated significant positive direct effects of age on diabetes (β = 0.409; p < 0.001) and smoking (β = 0.230; p < 0.001). Furthermore, smoking exerted a significant indirect effect on self-reported periodontal disease through psychosocial factors (β = 0.062; p = 0.043). In conclusion, psychosocial factors had a significant influence on self-reported periodontal status, highlighting the need for oral health strategies that are integrated with biopsychosocial support.
