CLINICAL EVALUATION OF THE USE OF ANTIBIOTICS IN SURGERIES TO INSTALL TWO STAGE DENTAL IMPLANTS: A CONTROLLED, BLIND AND RANDOMIZED CLINICAL TRIAL

SUMMARY:

Objective: to evaluate, through a controlled, randomized and triple blind clinical trial, the need for antibiotic prophylaxis, using 3 regimens of antibiotic therapy, in patients undergoing the installation of two-stage dental implants. Materials and methods: all patients signed an informed consent form. For randomization, the Microsoft Excel (2010) random function was used, which divided the groups at random and patients were allocated according to the list generated by the program. The research subjects were divided into 3 groups: group 1 (G1) formed by patients who did not use antibiotic prophylaxis, group 2 (G2) formed by patients who used preoperative antibiotic prophylaxis with amoxicillin 1 g orally 1 hour before the procedure and group 3 (G3), who used preoperative antibiotic prophylaxis with amoxicillin 1g orally 1 hour before the procedure and maintenance of the antibiotic, with 500 mg of amoxicillin for 05 days every 08 hours. All patients received the same protocol for pain and edema control, with doses of 8 mg dexamethasone and 500 mg of dipyrone sodium, 1 hour before surgery, both orally, with maintenance of sodium dipyrone every 6 hours in case of pain. Four postoperative evaluation periods were determined, being t1 (7 days), t2 (14 days), t3 (30 days) and t4 (120 days). The variables evaluated were: infection, pain (visual analog scale - VAS, and number of analgesics), wound dehiscence and exposure of the implant. In addition, at time t4, the implant was reopened for evaluation, implant mobility using manual torque, and bone loss. Results: a total of 60 patients were included in this study, totaling 114 dental implants evaluated. In order not to break the blindness until all patients have completed the four complete evaluation times, the results will be demonstrated descriptively, without association with the groups. The use of antibiotic prophylaxis in patients who undergo dental implants is still controversial, especially when they are two-stage surgical implants. In an attempt to answer the real need for antibiotic prophylaxis in two-stage implants regarding the clinical parameters of postoperative infection, pain and loss of implants until reopening, the results of this research are expected to try to standardize the clinical practice in implant dentistry regarding the prophylactic use of antibiotics in two-stage surgical implants.
